EDUCATION AND EXPERIENCE:

  • Minimum high school diploma or equivalent
  • Associate’s Degree preferred in early Childhood Education or health-related field preferred:

OR Previous parenting education or home visiting program experience

  • Experience using community resources, ability to advocate for others, and establish trusting relationship
  • Knowledge in maternal/child issues and development; experience working with or providing services to children and families

ALSO REQUIRED:

  • A current Illinois driver’s license; AND
  • Personal vehicle for use in carrying out required duties; AND
  • Auto liability insurance.

SKILLS, KNOWLEDGE AND ABILITIES:

  • Ability to complete Healthy Families Illinois training successfully
  • Ability to develop a close, trusting and supportive relationship with parents and families
  • Ability to identify families strengths
  • Ability to work with culturally diverse communities and families with humility and ability to be culturally-sensitive and appropriate; acceptance of individual differences
  • Ability to work with community agencies, including implementing referral process
  • Ability to work with families in a non-judgmental; way and to demonstrate sensitivity to their issues
  • Knowledge in personal computer use
  • Ability to maintain client confidentiality
  • Demonstrates good writing and organizational skills
  • Believes in and comfortable with advocating for nurturing, not-violent discipline of children
  • Willing to make a long-term commitment (at least 2 years) to the HFI program and its family/clients
  • Teaching, coaching, mentoring, role-modeling skills
  • Open to reflective practice (introspection, communicating awareness of self in relation to others, recognizing the value of supervision)
  • Has physical and mental health sufficient to meet the demands of the position
  • Strong commitment and ability to work as part of a team

MAJOR DUTIES AND RESPONSIBILITIES:

  • Communicates and coordinates goals and planned services with Family Case Manager
  • Attends weekly staff and supervisory meetings with Team Lead
  • Initiates and maintain home visits with families at regularly-scheduled intervals and as client’s needs require
  • Shares insight regarding family needs, identifies and implements workable solutions
  • Completes family assessment (FROG)
  • Observes and assesses infant/child developmental milestones
  • Makes use of community resources and referrals
  • Attends staff meetings as required
  • Documents all necessary information of the client record according to established protocols using the designated computer software system
  • Conducts outreach and community education activities

OTHER DUTIES AND RESPONSIBILITIES:

  • Assists with emergency and epidemic surveillance activities
  • Exhibits a commitment to exemplary customer service
  • Performs other duties as required or assigned

Pay: $16.00 - $16.35 per hour
