Health Care for the Homeless (HCH) is a program of New Horizon Family Health Services, Inc. (NHFHS) in Greenville, SC that sees patients experiencing homelessness in a multi-county Upstate area. Under the general supervision of the VP Clinical Services and Chief Medical Officer, and working with the HCH Program Manager, the HCH physician will be responsible for ensuring the quality of clinical services to program participants on the HCH mobile medical unit (MMU) and shelter sites . Primary duties will include assessment, diagnosis and treatment of patients experiencing homelessness and shall perform all professional duties in accordance with corporate policies and procedures as well as established standards of care.
Duties:
Works closely with the HCH Program Manager to ensure smooth and effective functioning of the HCH Program and MMU.
Participates in and actively supports the Continuous Quality Improvement (CQI) Committee and/or quality initiatives to ensure compliance with the HCH CQI Plan.
Effectively provides clinical services and comprehensive services including assessment, diagnosis, appropriate and effective treatment and referral.
Effectively provides quality primary care, preventive care, acute care, and chronic disease management based care based on established standards of care.
Actively participates on an integrated, multidisciplinary team to develop and implement an integrated treatment plan, within the context of individual or family's unique social needs, as well as contributes to interdisciplinary case conferences on an ongoing basis.
Actively participates in advocacy activities that support NHFHS’ mission and the HCH Program.
Evaluates patient problems thoroughly and appropriately.
Effectively, clearly, and accurately documents patient visit notes, orders, telephone calls, clinical flow sheets and other records in the Electronic Health Record as required by quality standards.
Demonstrates excellent communication skills and effectively communicates with patients, families, and other staff members.
Demonstrates sound judgment and decision-making skills and practices within his/her scope of expertise and licensure.
Consults with other physicians appropriately: knows when to seek advice, Refers patients requiring inpatient services to Teaching Services or Hospitalist Service at the patient’s hospital of choice, performing appropriate hand-off of care and following up with the patient after discharge from hospital.
Demonstrates teamwork by working effectively with all team members, attending and participating actively in group meetings, and conferring with practice managers and/or clinical managers (i.e. lab, pharmacy, nursing) about any issues that arise.
Responds to MMU emergencies, chart completion, forms completion, peer review, and other needs in a cooperative fashion.
Consistently demonstrates professional demeanor and appearance. Serves as a good role model for staff and students.
Stays current in the professional field and follows principles of evidence-based medicine. Follows clinical guidelines adopted by the Corporation.
Establishes rapport with public and private clinical care referral resources.
Actively participates in and comply with all aspects of the NHFHS Corporate Compliance Program, HCH CQI Plan, and follow the Program Code of Conduct and obey all relevant laws, statutes, regulations, and requirements applicable to Medicaid, Medicare, and other State and Federal Health Care Programs.
Observes schedule for work, breaks and lunch, responds cooperatively to changes in schedule and work assignments. Demonstrates flexibility in meeting patient care needs outside regular working hours.
This is a part time position.

Experience and Skills:
Education:
Must have successfully completed medical training at a four (4) year, accredited Medical School and received an M.D. or D.O. degree.
Licensure/Certification:
Must be Board-eligible, become Board-Certified in Family Medicine or Med/Peds or Internal Medicine within 1 year of initial hire date and continue to maintain Board Certification for the length of employment with NHFHS.
Must be BLS CPR certified
Must be licensed to practice Medicine in South Carolina.
Knowledge and Skills
Extensive knowledge of the techniques and skills necessary to diagnose and properly treat diseases and injuries.
A working knowledge of the administrative practices and procedures, rules, regulations and policies that govern the rendering of responsible medical and nursing care.
A working knowledge of public health and preventative care guidelines.
Excellent communication skills.
The ability to relate with warmth and compassion to patients and co-workers.
The ability to effectively supervise professional and non-professional employees in their clinical duties.
