Position Summary

Provide and direct Physical Therapy services to residents to include assessment and treatment, development and Implementation of programs, and related professional in-services. Preforms job duties under the directions of physician orders.
Purpose
Restores resident's function, alleviates pain, and prevents disabilities by planning and administering medically prescribed physical therapy

Essential Duties and Responsibilities
Provides Physical Therapy services to residents and initiates referrals when appropriate and in accordance with facility policy and procedures and Towne Square Care policy and procedures
Conducts screening of residents at regular intervals to determine need for intervention/treatment
Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated
Interprets and communicates evaluation findings to residents, family and other team members
Provide PT services at other Towne Square Care sites as needed
Establish individual resident schedules in collaboration with resident and other team members
Develops treatment plans by identifying a problem list, including short and long term goals and methods to achieve the identified goals
Coordinates treatment plan with resident, family and other team members Implements and conducts treatment as outlined in treatment plan
Monitors residents' response to intervention and modifies treatment as indicated to attain goals in accordance with physician's orders
Provides, recommends, fabricates adaptive devices and other equipment; trains resident, caregiver, and other team members in the use or applications of orthotics and prosthetics

Participates in discharge planning;-including a home evaluation if indicated and appropriate.
Develops appropriate home-or community programming to maintain and enhance the performance of the resident in their own environment
Documents results of resident assessment, treatment, follow-up, and termination of services in a timely manner in accordance with Towne Square Care documentation policy
Supervises and directs LPTAs in the implementation of the treatment plan
Supervise students and volunteers in accordance with state regulations
Participates in resident, family and staff education
Attends in-service meetings and facility meetings as required
Provides in-service education to members of residents care team and education to the community
Consistently demonstrates sound judgment in the evaluation, planning, and implementation and follow-up of resident therapy programs
Assumes responsibility for necessary ongoing continuing education and professional development
Accurately records treatment time and other non-billable time per Towne Square Care policies
Provides treatment in accordance with established standards of practice, department procedures, state practice acts, federal regulation, APTA guidelines and productivity standards
Reports.to work on time and adheres to a work schedule that maintains acceptable patient ratios and work efficiency during assigned shift
Maintains a clean and safe work area in adherence to facility and Towne Square Care standards
Complies with all Infection Control, Universal Precautions and OSHA standards for the health care professional
Maintains positive relationship and rapport with coworkers, residents, family members and facility personnel
Assists with and participates in Quality Assurance Programs as required
Adheres to establish confidentiality and HIPPA standards
Projects a positive and professional image at all times
Performs other duties as assigned

Recommended Minimum Position Qualifications
Bachelor of Science / Master Degree in Physical Therapy from an accredited program Successful completion of National Certification Examination for Registered Physical Therapist

Pay: $35.00 - $60.00 per hour
