About the Role:
As a Pharmacy Care Navigator, you'll collaborate with comprehensive interdisciplinary roles as part of a managed care team to minimize gaps in treatment and eliminate barriers to medication adherence. You will provide members with medication, access, adherence and affordability resolution services, identifying and communicating discrepancies to build long-term solutions. You'll connect Cityblock members and care teams to pharmacy resources and support enrollment in qualifying copay assistance programs. This position acts as a medication therapy liaison and navigator for the member, the Cityblock care team, external providers, and the health plan.
You will:
- Work in a radically different model of healthcare, collaborating with a multidimensional care team
- Collaborate with clinical and non-clinical care team members, including our large team of Community Health Partners
- Co-manage a panel of members to improve their health outcomes through longitudinal primary management, and care coordination. You will meet with members virtually and by phone, to help obtain a list of home medications and other pharmacy-related information from members, caregivers, pharmacies and/or providers.
- Identify and communicate medication discrepancies to both internal/external providers and the pharmacist for intervention. .
- Review medication list in EHR to understand and help identify barriers to medication therapy adherence
- Work collaboratively with CB care team/external providers to help eliminate all identified barriers through interventions, including but not limited to:
- Serve as the Pharmacy team's first line of triage for care teams and members with pharmacy and medication related needs, problems and questions.
- You will be addressing incoming queue referrals for medication access, adherence & affordability support including, but not limited to:
- Prior authorizations, PA appeals, copay assistance, resolving rx billing issues with the pharmacy, blister packing and delivery coordination, refill requests etc.
- Assist with admin support, such as obtaining medication list clarification, identifying discrepancies, routing documentation and alerting the pharmacist of discrepancies as per clinical protocol, post visit tasks, follow up, scheduling, and obtaining additional documentation as assigned for the Comprehensive Medication/Targeted Disease Management program if additional support is requested by CB clinical pharmacists
- Review medication list in EHR to understand and help identify medication discrepancies, gaps and barriers to adherence, whether a pharmacist clinical intervention is needed and routing for review, and escalations as required while keeping the care team informed and updated.
- Communicate with internal/external providers, care teams, pharmacies and both clinical + non clinical staff to help obtain and clarify missing clinical information for our members and/or coordinate medication access related services.
- Coordinate set up of med synchronization, blister packaging, pharmacy transfer, home delivery, or mail order services.
- Host weekly forum for team members to join for Q+A on non-clinical, medication and pharmacy related questions.
- Assess member's eligibility for available copay and foundation assistance programs to lower medication costs.
- Sustain a general knowledge of available patient assistance programs, foundations, cards/programs for copay assistance and help members complete applications for enrollment.
Requirements for the Role:
- You have a minimum 6 years experience as a pharmacy technician in a retail and/or outpatient pharmacy
- You have a valid Pharmacy Technician Certificate (CPhT) from PTCB.
- NYS Pharmacy Technician Registration or ability to obtain within 6 months of start date.
- A valid medication history certificate from PTCB or ability to obtain within 60 days of start date
- Experience actively compiling an accurate and reliable medication history and performing medication reconciliation in the EHR.
- You have a lot of experience communicating with patients, social care workers, providers and pharmacists on medication, and third party plan issues.
- Prior experience with and knowledge of medical terminology, ICD-10/CPT codes, 3rd party billing resolution, including Medicaid/Commercial and Dual eligible plans, prior knowledge of pharmacy insurance prior authorization process and experience with medications.
- Ability to successfully engage members, close all identified gaps and medication management opportunities, and meet defined market team success metrics and goals. .
- You have a working knowledge of pharmacy claims billing and medication access resources.
- You excel at empathy and human interactions, and want to improve the health of individuals and whole communities.

We take into account an individual's qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, retirement benefits, participation in the company's equity program, paid time off, including vacation and sick leave, and any other tax-reportable benefits. The expected salary range for this position is $37,179.00 to $40,050.00. The actual offer will be at the company's sole discretion and determined by relevant business considerations, including the final candidate's qualifications, years of experience, skillset, and geographic location.

You must complete Cityblock's medical clearance requirements, which include, but may not be limited to, evidence of immunity to MMR, Hepatitis B, Varicella, and a TB screen, or have an approved medical or religious accommodation that precludes you from being vaccinated against these diseases.
