Job description
The Perinatal Health Educator works as a member of a multidisciplinary Comprehensive Perinatal Services Program (CPSP) team which includes social workers, dietitians, medical assistants, medical practitioners including midwives and physicians, and other clinic staff to provide comprehensive care in a non-judgmental manner. Health education is part of a comprehensive approach to reducing the incidence of low birth weight babies. The Clinics emphasize prevention and health education in all services. In addition, the PN Health Educator provides administrative support.
CommuniCare Health Centers is a federally qualified health center providing comprehensive patient-centered primary care services including medical, dental, substance abuse treatment, mental health and outreach services to low income, multi-ethnic, uninsured and underinsured populations.
SCHEDULE: 32 hours per week, Monday (12pm-9pm) and Tuesday-Thursday (8am-5pm) at Hansen Family Health Centers in Woodland. One evening per week required. Travel between sites in West Sacramento and Davis as needed.

RESPONSIBILITIES:
The following reflects managements definition of essential functions for this job, but does not restrict the tasks that may be assigned. Management may assign or reassign duties and responsibilities to this job at any time with reasonable accommodation.
- Provide one-on-one Comprehensive Perinatal Services Program (CPSP) services including: initial patient orientation, financial screening, assessment of psychosocial, nutrition and health education needs and care plan development based on needs. Perform reassessments at each trimester and at two weeks postpartum with care plan updates as needed. Provide perinatal health education throughout patients pregnancy.
- Consult and refer per CPSP guidelines to Social Worker, Nutritionist, Teen clinic, Dental Clinic, etc.
- Timely and complete chart documentation as required by CPSP.
- Review charts prior to clinic to identify clients in need of CPSP services and/or health education.
- Track patient case load to ensure patients are scheduled and attending reassessment visits, education visits, classes and referral appointments.
- Provide education on a variety of pregnancy and postpartum topics per CPSP guidelines.
- Attended National Child Passenger Safety Certification training to provide car seat education to prenatal patients.
- Attend monthly staff meetings and trainings.
- Other duties as assigned.
MINIMUM POSITION REQUIREMENTS:
Education & Experience: College degree or high school diploma and college coursework. Experience in a medical office, clinic or hospital preferred. Experience in prenatal and postpartum care or Womens health services is preferred. Demonstrated ability to work with low income populations.
Special Skills/Equipment: Bilingual Spanish/English required. Must provide proof of current drivers license and auto insurance. Effective communication skills and ability to encourage patient understanding of prevention practices.
Expectations: Attendance & punctuality is necessary. Required to have versatility, flexibility & a willingness to work within constantly changing priorities with enthusiasm & have acute attention to detail. Expected to have strong interpersonal skills & have the ability to understand and follow written/verbal instructions. Required to have the ability to deal effectively with a diversity of individuals at all organizational levels and with external customers.

Must be able to sit or stand for long periods of time, with occasional walking. Hearing is required for patient assessment. Vision required to read medical charts & reports. Must be able to lift & carry up to 25 lbs. COMPLETE COVID-19 VACCINATION REQUIRED
PAY RATE:
- The pay range for this position is $20.30/hr.-$28.20/hr. Starting pay within this range will vary depending on the job-related knowledge, skills, and experience.
- May be eligible for annual 2% + 0-2% merit-based pay adjustment (0-4%).
