Job Summary and Qualifications

SPECIFIC ELEMENTS AND ESSENTIAL FUNCTIONS*

  • Initiates denial and appeal process in accordance with policy for current Rehab patients
  • Identifies and intervenes to coordinate care on patients in relation to appropriateness of level of care length of stay and resource utilization issues
  • Provides initial and ongoing psychosocial assessments for patients/families having complex psychosocial needs and/or medical complex diagnosis. Ensures patient is progressing towards desired outcomes and identifies barriers that hinder effective patient care.
  • Documents comprehensive social work assessments, treatment goals, interventions, progression and discharge plans in the patient’s medical record congruent with departmental policies.
  • Demonstrates leadership in the professional development of self and others
  • Communicates and works effectively with patients, family members/significant others, and other healthcare professionals.
  • Participation in the interdisciplinary treatment planning process, attending team meetings, and working in conjunction with the treatment team.
  • Performs concurrent review for all payers and maintains documentation as directed by the Rehab Department.
  • Identifies, tracks, and records Avoidable Care Days. Intervenes to prevent or decrease ACD as appropriate.
  • Provide weekly updates, education, and encouragement to patient/family
  • Provides crisis and supportive counseling to enhance the patient’s and family’s problem solving and coping skills .
  • Identifies and resolves barriers that hinder effective patient care
  • Facilitate collaboration with patients existing sources of support; identify new sources of support
  • Educates patients and families regarding post discharge needs and provide resource information
  • Coordinate discharge planning activities
  • Initiate community/facility referrals based on identified patient/family needs.
  • Collaborates with community agencies to provide services to assist the needs of patients/families post discharge.
  • Consistently work 2-3 shifts per week; 8 hour shifts

QUALIFICATIONS:



EDUCATION

Required: Master’s Degree in Social Work or Human Service Field

Preferred: Current Licensure by the Common Wealth of Virginia as a clinical social worker

EXPERIENCE

Required: Experience in discharge planning or case management

Preferred: Two years’ experience in a hospital or health care setting
