BRIDGES MN HAS ORIENTATION FOR EVERY MONDAY ! We offer 1st, 2nd, and 3rd shift opportunities available full-time or part-time, Sunday through Saturday. Salary: $55,000- $60,000
Job Roles/Responsibilities: (Essential Functions)
Engage in outreach to partners and other programs to promote Bridges MN services
Attend industry training, events and networking
Keep up to date with market trends and industry policy to ensure Bridges services remain in compliance and ahead of the curve
Field calls and emails from referral sources/clients
Filter/pre-qualify opportunities and discuss services to ensure there is an opportunity for success
Develop and maintain a system of tracking referrals, potential clients, residential placement, caseload, and a running census
Schedule tours of housing
Coordinate engagement with site supervisors and director team if necessary to ensure proper placement
Screen individuals, identify initial goals and service needs
Propose services to support client needs
Review any impacts with team/resident on benefits if applicable (DB101.org)
Track and ensure the accuracy of service agreements
Build client electronic records, request relevant client information and support team on the transfer of information through the intake
Coordinate moves and send out move in coordination document to relevant parties
Support supervisors throughout the intake and ensure a successful placement
Follow up after move in through transitions
Instruction and assistance to direct support staff implementing the CSSP an the service outcomes
Work with Development Director on caseload balancing to ensure appropriate staffing for current and future placements
Complete all 245D Service documentation for intake-45 Day
Assist supervisors as needed on any resident discipline or engagement issues during first 45 days
Engage in 45 day meeting to support supervisors
Ensure all documentation is completed by 45 day meeting both at the site and electronically (intake, 245D and CSSP Addendum)
Ensure corrective action is taken when ordered by the commissioner and that the terms and conditions of the license and any variances are met
Evaluate the information identified in clauses (1) to (6) to develop, document, and implement ongoing program improvements.
Required Skills and Experience (Minimum requirements in terms of educational background, work experience, licenses/certifications or other knowledge skills and abilities)

Strong organizational, problem-solving, and analytical skills
The Designated Manager must minimally have:
(1) a bachelor's degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
(2) an associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
(3) a diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
(4) a minimum of 50 hours of education and training related to human services and disabilities; and
(5) four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3)
(6) a minimum of three years of supervisory level experience in a program providing direct support services to persons with disabilities or persons age 65 and older
