The Arc of Chemung-Schuyler is currently seeking a Clinical Social Worker for the Bath, NY area.
GENERAL DESCRIPTION: What will I do?
Responsible for providing counseling supports to individuals to improve personal, interpersonal, and environmental interactions which interfere with attaining maximum independence and community inclusion.
MINIMUM QUALIFICATIONS: What will I need?
Master’s Degree from an accredited school of Social Work and NYS Licensure as a Licensed Master Social Worker. Licensed Clinical Social Worker preferred. Experience in a human service setting supporting individuals with developmental disabilities is also preferred. Must maintain licensure status. Experience or knowledge of Windows, Internet, Excel, Access, Word, PowerPoint, and Email preferred.
DUTIES AND RESPONSIBILITIES: Tell me more!
1. Completes assessments on behalf of individuals, designed to evaluate social, economic,
environmental and disability factors which interfere with a person’s goal to attain maximum independence and community inclusion. As assigned, completes specialized assessments or evaluations in such areas of suicide/homicide risk, ability to consent to sexual activity, forensics, community safety/risk and ability to understand and appropriately exercise individual/human rights.
2. Develops and implements a Treatment Plan designed to address factors interfering with the individual’s interests, goals, or ability to fully participate in the community. On a semi-annual basis, reviews the status and progress of the Treatment Plan with the individual. Provides results of such review (i.e.: six-month summary) to the DHS Treatment Coordinator.
3. Provides individual and/or group counseling supports consistent with the individual’s interests and goals and as defined in the Treatment Plan, and in accordance with accepted treatment standards. Works with family members, significant others, and service providers, as necessary to facilitate, success of clinical supports. Adjusts counseling technique and style to meet individual cognitive and personality traits.
4. Documents all clinical activities in the confidential clinical record (e.g.: progress notes, case notes, assessments, six-month reviews). Documentation is completed contemporaneously to the date of encounter.
5. Participates in in-services and conferences designed to enhance work skills and encourage professional growth. Researches innovative and evidence-based treatment methods and implements accordingly.
6. Maintains and adheres to ethics of confidentiality and in accordance with HIPAA and/or mental hygiene requirements.
7. Performs other duties as assigned by the Developmental Health Services Director.
