A Bachelor’s degree in Nursing, an Associate’s degree in Nursing, or graduation from a Diploma Nursing Program AND three (3) years of casework or social work experience in a long-term care or medical facility OR a Master’s Degree in Nursing AND two (2) years of casework or social work experience in a long-term care or medical facility.
Pre-Employment Covid-19 Vaccination and Background Check Requirements:
It is the policy of Allegheny County that in order to receive a final offer of employment, candidates must successfully pass a background check, which includes verification of Allegheny County Real Estate Tax status, and drug/medical examinations as required for the position. The pre-employment drug screen must be completed within 48 hours of the acceptance of the conditional offer of employment.
Additionally, it is the policy of Allegheny County that in order to receive a final offer of employment, candidates for positions within the Kane Community Living Centers must provide proof of a Covid-19 vaccination. The prospective employee will need to be fully vaccinated or have at least one shot of a two-shot series before beginning work. Any employee who does not receive and provide proof of the second shot within 30 days will have their probationary employment terminated. Allegheny County will allow for medical and religious exemptions from the requirements in accordance with state and federal law.
Position Summary:
The incumbent serves as the marketing/outreach contact for all ongoing admission efforts for the Kane Community Living Centers system. Work is performed in accordance with Federal, state and local standards, guidelines, and regulations that govern long-term care facilities, and under the direction of the Admission Services Manager.
Duties:
- Maintains and updates data base for all personal care, assisted living, hospital and rehabilitation centers and other health care related referral sources.
- Assists artists and print contractors with the layout and design of newsletters, reports, and other promotional materials.
- Participates in community meetings and activities to explain or provide information on the Kane Community Living Centers activities, services, and initiatives.
- Develops operational plan for ongoing recruitment of residents and review strategic data for indications of residency trends and needs.
- Visits and attends meetings outside of facility to promote the Kane Community Living Centers.
- Conducts thorough assessment of applicant from phone inquiries and onsite assessment of applicant when appropriate.
- Provides specific information regarding the admission process or status to families, consumers, community hospitals, community agencies, etc.
- Reviews application forms, completes pre-admission assessment form determining appropriateness for admission, and coordinates admission to Kane Community Living Centers.
- Assists Allegheny County Area on Aging with emergency and respite admissions.
- Communicates with outside agencies, facilities, etc. such as Medical Assistance Eligibility Verification System and Community Hospital Medical Records Department to verify hospital stays.
- Prepares suggestions and ideas, and assists in the design of promotional ads, brochures depicting the Kane Community Living Center’s caregivers, programs, or services.
- Performs related duties as assigned or directed.
Knowledge, Skills, and Abilities: Knowledge of:
- Laws, regulations, and guidelines that govern long-term care.
- Medicare, Medical Assistance, HMO’s, and third-party insurance.
- Programs, services, activities of long-term care facilities.
- Design and layout methods and techniques for print materials.
Ability to:
- Work with ill, disabled, elderly, and emotionally upset people within a long-term care facility.
- Plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures related to the admissions policy of the Kane Community Living Centers.
- Function independently, and have flexibility, personal integrity, and work effectively with applicants, residents, personnel, and support agencies.
- Communicate effectively both orally and in writing.
