PRIMARY RESPONSIBILITIES:
The Wraparound Facilitator Specialist coordinates services, supports and advocates for the family and connects resources to the family. It typically involves assessing family needs, connecting with community resources, and acting as a service coordinator for the family unit. Knowledge of best practices, working cooperatively with other agencies within the system of care framework, family friendliness and wraparound philosophy are a necessity. The Wraparound facilitator partners with families whose children are at risk of out of home placement. The Wraparound facilitator helps the family identify their strengths and rallies the extended family and natural supports to create a unique, family-driven plan of action to keep the children where she/he belongs…safely at home. We need someone who sees possibilities, not problems.
ESSENTIAL DUTIES
:
- The Wraparound Facilitator Specialist will maintain of a caseload of approximately 12 youth/families which includes all identified youth and a limited number of families who may not utilize residential services but need short- term intensive case management. Facilitate monthly interagency planning meetings for
- The Wraparound Facilitator Specialist will be responsible for creating a community support plan and/or discharge plans for all families who have children receiving services from Hope Haven. Provide outreach and emergency services and counseling to families in a crisis. Assist families in crisis in finding professional and informal support systems.
- The Wraparound Facilitator Specialist will maintain accurate documentation on all activities in a timely manner. This includes, but is not limited to case notes, assessments, service plans, case staffing notes, and weekly/monthly reports.
- The Wraparound Facilitator Specialist will maintain confidentiality both in writing through signed release of information forms and in day-to-day interactions with community resources and service families. This will include obtaining information from other service agencies on the consumers served.
- When assigned, will work with HHHB Therapeutic staff to serve a family’s wrap/support needs and to facilitate service coordination if requested by the family. Create and maintain a team of system partners and natural supports for each adolescent and family. Identify and link adolescents and families to community resources
- The Wraparound Facilitator Specialist will develop and maintain contacts with other agencies to better facilitate the creation of service teams. Will serve on youth’s services teams if there is a need identified by the team and the family.
- The Wraparound Facilitator Specialist will provide transportation as needed for the families we serve or will make arraignments for the families’ transportation to psychiatric appointments.
- The Wraparound Facilitator Specialist will provide information and guidance to the family system to help them meet their own needs in the future.
- The Wraparound Facilitator Specialist will be responsible for gathering data on all assigned cases. This may involve assisting the HHHB Therapeutic staff with enrolling youth from their caseload into assigned MYPAC services.
- Wraparound Facilitator Specialist will maintain 20 regular consumer contact hours weekly as well as other assigned statistical information. Attend at least 3 hours of group or individual coaching/supervision per week
- Upon employment, the Wraparound Specialist will complete the HHHB trainings that focus on Cognitive behavior therapy to familiarize him/her with the type of trauma treatment used in HHHB.
- Follow the 4 phases of High-Fidelity Wraparound and all activities associated with each phase Adhere to the 10 Principles of High-Fidelity Wraparound and ensure that the Principles and steps of the High-Fidelity Wraparound process are delivered with the highest possible fidelity.
- Other non-essential duties as specified by the immediate supervisor.
EXPERIENCE / KNOWLEDGE
:
- Bachelor’s degree and licensure or certification in social work and or counseling required.
- Two years of post-college experience providing direct care with crisis intervention experience to children and families.

- Excellent written and oral communications skills
- Must possess skills to be empathetic, nonjudgmental, supportive, respectful, and empowering in working with victims of interpersonal violence
- Excellent analytical and abstract reasoning skills
- Ability to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.
- Ability to respond to crisis situations effectively and appropriately.
- Ability to work collaboratively with agency programs and organizations outside the agency.
- Ability to work a flexible work schedule to meet program needs
- Proficient with Microsoft Word and Excel
- Ability to handle multiple projects simultaneously, work both independently and in a team environment.
- Ability to work with minimal supervision and meet stringent deadlines.
LICENSES/CERTIFICATIONS:
- Bachelor’s Degree in social work, sociology, psychology, or other human service-related fields.
- WV Driver's Licenses (Required) and access to a vehicle.
- Maintenance of adequate vehicle insurance.
- Work authorization: United States (Required).
PHYSICAL/MENTAL REQUIREMENTS:
- Must be able to read, write, and speak English.
- Manual dexterity and motor skills are required to use the computer.

AGENCY COMPLIANCE
:
- Refers to and complies with the CCI Policy and Procedural Manuals in every part of their job.
- Complies and adheres to assigned work schedule.
- Adheres to the agency’s confidentiality policies, maintaining a high-level standard of confidentiality
- Attends individual supervision, staff meetings, staff trainings and other meetings required by agency, funding sources and/or professional standards.
- Enters all daily client services/activities into the agency client data base.
- Maintains all required documents in a current and orderly manner.
- Submits and ensures all required agency and grant programs statistical and narrative reports are done so accurately and promptly per the prescribed due dates.
- Represents the agency at community and funder events/function as requested.
- Maintains a valid driver license, proof of required automobile insurance, and Child Protection requirements per agency policy.
- Always be alert to the physical and mental condition of clients and children and report any suspicious situations to the Associate Director.
