JOBWHAT WE OFFERThe County of Ventura offers an attractive compensation and benefits package. In addition to the salary of approximately $83,613.94 to $125,584.38 annually an employee within this position will also be eligible for the following: Educational Incentive - An educational incentive of 2.5% for completion of an associate's degree, 3.5% for completion of a bachelor's degree, OR 5% for completion of a graduate's degree. Bilingual Incentive - Incumbents may also be eligible for bilingual incentive depending upon operational needs and certification skills. Annual Leave - 208 hours per year, increasing to 288 hours after 5 years of service. Annual Leave Redemption - The ability to "cash in" or redeem up to 100 hours of Annual Leave per year after using 80 hours in the preceding twelve months. Deferred Compensation - Eligible to participate in the County's 401(k) Shared Savings Plan and/or the Section 457 Plan. This position is eligible for up to a 3% match on your 401(k) contributions. Health Plans – Full time employees are afforded a flexible credit allowance for purchasing medical, dental, and/or vision insurance from a group of authorized plans: Tier one for employee only is $497 per biweekly pay period, tier two for employee plus one is $622 per biweekly pay period and tier three for employee plus family is $737 per biweekly pay period. Flexible Spending Accounts - Choice of participation in the Flexible Spending Accounts which increase spending power through reimbursement of pre-tax dollars for IRS approved dependent care and health care expenses. Pension Plan - Both the County and employees contribute to the County's Retirement Plan and to Social Security. If eligible, reciprocity may be established with other public retirement systems, such as PERS. Holidays - 11 paid days per year which includes a scheduled floating holiday. VENTURA COUNTYThe County's 1,873 square miles include 1,846 square miles of land with 42 miles of coastline which is home to the following cities and points of interest: Camarillo, Fillmore, Moorpark, Ojai, Oxnard, Port Hueneme, Santa Paula, Simi Valley, Thousand Oaks, and Ventura as well as national protected areas such as parts of the Angeles National Forest, Channel Islands National Park, and the Santa Monica Mountains National Recreation Area. Ventura County is a “general law” county, governed by a five-member, elected-by-district Board of Supervisors. The Supervisors appoint a County Executive Officer (CEO) to oversee the County budget, day-to-day operations, and to advise, assist, and act as an agent for the Board of Supervisors in all matters under the Board’s jurisdiction. THE DEPARTMENTThe Ambulatory Care Department is a system of primary care and specialty care medical clinics conveniently located throughout the County of Ventura. In keeping with the mission of the Ventura County Health Care Agency, our team of dedicated physicians, nurses and other health care professionals seek to set the standard in health care excellence with the goal of having healthy people in healthy communities throughout Ventura County.THE POSITION Under general direction of the Director, Complex Care Coordination and Systems Integration, the Whole Person Care Program Managers facilitate and oversee the development and implementation of new models and systems related to the CalAIM Program which allow for greater patient care, improved health outcomes of target populations and greater cost effectiveness of healthcare services.Working across County agencies and departments, the Whole Person Care team implements the CalAIM Enhanced Care Management and Community Supports programs which works to improve care for people with the most complex needs: behavioral health needs such as serious mental illness, emotional disturbance, and/or substance use disorder; seniors and people living with disabilities; people experiencing homelessness who also have complex physical or behavioral health needs; people transitioning from jail or prison back to the community; children and youth with complex medical conditions and/or in foster care. CalAIM Community Supports substitutes new clinical and non-clinical services for traditionally covered services, such as care in a nursing home or hospital, to meet needs in new, more patient-focused ways. EXAMPLE OF DUTIES
Duties may include but are not limited to the following: Performs duties and tasks in support if the daily operations of the WPC team, implementing CalAIM enhanced care management and community supports that coordinate care across departments and agencies;Facilitates and oversees the development and maintenance of models and systems of care for the Whole Person Care (WPC) team in support of the CalAIM Enhanced Care Management and Community Supports programs which allow for greater patient care, improved health outcomes of target populations and increased cost effectiveness of healthcare services;Develops policies and procedures, workflows, and team processes;Participates in educational outreach, engages stakeholders, and develops collaborations with outside community organization;Develops and implements a collaborative identification process for CalAIM populations of focus with complex care needs, including people with multiple chronic conditions, people experiencing homelessness, people in need of specialty mental health care and/or substance use services, people who are aging and in need of long-term care, people who have been recently incarcerated, pregnant mothers and children and youth;Identifies opportunities for improvement and greater efficiency by performing gap analysis and related studies;Oversees the collection, analysis, validation, and reporting of clinical data establishing performance metrics to assist healthcare leaders with decision support and program evaluation;Represents the department, agency and county in meetings, conferences, workshops, outside functions, community events and other forums;Manages development and implementation of care coordination efforts, including technical infrastructure for data-sharing to facilitate improved outcomes;Facilitates interagency workgroups and project-based teams;Confers and collaborates with other managers on policy matters and work challenges;Performs other related duties as required.
MINIMUM QUALIFICATIONS

Substitution: Additional qualifying experience may be substituted for the required education on a year-for-year basis up to a maximum of four (4) years.DESIREDMaster's degree in accounting, finance, business administration, health care services administration, mental health, public health, or a related field,Experience: Working with populations of focus served by CalAIM,Designing programs that address social determinants of health, such as housing services, recuperative care, medical meals, personal care, respite care, etc.Working with Medicaid Waiver programs.Knowledge, Skills, and AbilitiesConsiderable to thorough knowledge of: principles of supervision and management; principles and techniques of fiscal management including budgetary preparation and analysis and detailed pro forma; principles and techniques of organization and staffing; application of office automation to business processes; contract negotiation; marketing of services both internal and external to the County; public relations; principles, practices, procedures, and techniques involving the collection, analysis, interpretation, and reporting on statistical data; principles and practices of data administration and workflows in a clinical setting; principles and techniques of effective project management, including organizing and managing the flow of work in a collaborative environment; and federal, state and local laws, rules, regulations, policies, processes, and trends affecting the healthcare environment, including HIPAA, 45 CFR Part 160 and Subparts A and E of Part 164, California Civil Code Section 56, and the special terms and conditions for the CalAIM 1915b Medicaid Waiver.Skilled in: performance measurement and program evaluation; implementing quality improvement and change management activities; operating computers, office applications and electronic health record systems; project management tools and other various software, such as Microsoft SharePoint, Excel, Gantt charts, Tasks, Access, etc.Working ability to: analyze administrative and organizational problems and recommend effective solutions; prepare a variety of reports and recommendations; communicate effectively in an oral and written manner; plan, organize and supervise the work of others to meet challenging, changing priorities and deadlines; develop and present training programs for agency/department staff and others; quickly learn and master a variety of software and project management tools; establish and maintain effective working relationships; recognize and nurture opportunities for collaboration across departments and community stakeholders, such as challenging populations including the seriously mentally ill, seriously emotionally disturbed and adults with chronic illnesses.
