Personalized Nursing LIGHT House is a premier behavioral health care provider with 32 years of specialized addiction treatment service serving the adult population. We have locations in Plymouth, Canton, Lakeland (near Brighton), Dearborn Heights, Madison Heights and Farmington. All services will integrate substance use disorder, mental health and primary health care treatment in cohesive and client-centered programs.

POSITION DESCRIPTION

We are seeking candidates to work at our Madison Heights and Plymouth, Michigan locations which are adult outpatient treatment centers. We are looking for individuals to identify, screen, assess, and develop service delivery treatment care plans for persons recovering from substance abuse. Direct clients to needed services, coordinate and monitor access to and delivery of quality services, and provide client advocacy when necessary. Participate in team conferences with other professionals. Assist in ensuring that appropriate services, including health care, psychological, financial, and legal, are provided to clients at the most affordable costs. Candidates will have counseling experience in Substance Use Disorder with co-occurring disorders. Excellent and efficient reporting skills a must.

These are full time positions working both days (9:00 am - 5:30 pm Monday-Friday).

RESPONSIBILITIES

1. Use the LIGHT Model in all counseling interactions.

2. Utilize the LIGHT Model in the assessment, teaching, and counseling of clients (one-on-one or in groups).

3. Develop with client goal-oriented, time-limited, and measurable goals based on identified needs, available community resources, and financial status of client.

4. Monitor and coordinate through contact with client/family and service providers to ensure optimal utilization of offered services and adequate provision of needed services.

5. Reassess goals with client to evaluate the effectiveness of plan in meeting pre-determined goals. Revise goals and/ or plans accordingly.

6. Establish and maintain up-to-date client records, including demographics, assessment, service plan implementation, and evaluation of care.

7. Maintain client confidentiality.

8. Participate in multi-disciplinary case conference meetings.

REQUIREMENTS

A Master's degree in a human services field (social work, psychology, criminal justice, etc.), with licensure (LLMSW, LMSW, LLPC, LPC) with MCBAP certification (CADC) or approved development plan. At least two (2) years substance abuse counseling experience is also required.

Pay: $20.00 - $25.00 per hour
