Responsibilities

• Skilled Visits
• Day-to-day patient interaction
• Performing Start of Care Assessment for potential home health patients
• Education on Health Conditions, such as CHF, COPD, Diabetes
• Performing wound care, wound vac proficient, it not proficient HHA can teach
• Medication Management
• Changes dressings, bandages and contraptions as prescribed
• Documentation of patient’s condition and pointing out significant changes, to be reported to the physician
• Working along side other nurses, Physical Therapist, Speech Therapist and/or Occupational Therapist

Requirements

• Kansas or Compact Nursing License
• On call
• Strong organizational and self-management skills
• Valid driver’s license
