SIGN-ON BONUS: $4,000

We are seeking a full-time RN Training Coordinator to facilitate personal care tasks training to new hires Tuesdays and Thursdays (8:30a-5p) in our Woodbury Office. This position will also be responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, supervising HHA's and CNA's, and serving as a backup support to their peer Training Coordinator.

Job Duties

  • Train new hires in company policies and procedures, evaluating engagement and participation
  • Conduct shadow shifts, unannounced visits, and attend weekly meetings as requested
  • Collaborate with manager to ensure effective performance, development, and retention programs
  • Conduct initial home health care OASIS assessments for comprehensive care, including Starts of Care, Resumptions of Care, and Recertifications, maintaining 32-40 productivity units
  • Perform skilled nursing tasks as assigned under the direction of the Clinical Manager and CareAparent scheduling
  • Oversee and supervise Licensed Practical Nurse (LPN) visits and ensure plan of care is being followed
  • Report changes in client condition to physicians and Clinical Manager
  • Prepare appropriate medical documentation of client assessments and visits using CareAparent EMR platforms
  • Perform other nursing functions as may fall under state and federal nursing guidelines, CMS, and the Joint Commission
  • Able to work on call for rotating evening, weekends, and holidays
  • Be self-directed to manage an effective and productive caseload of patients and clients.
  • Perform other skilled nursing visits as directed by the Clinical Manager or Clinical Manager
  • Other duties as assigned

Education and Licenses

  • Graduate from an accredited and approved nursing program
  • Valid Minnesota RN License

Work Experience

  • Minimum of one (1) year of relevant home care or assisted living experience
  • Supervisory/management and training experience preferred

Required Skills, Knowledge, and Abilities

  • Must possess positive, enthusiastic attitude, good people skills, and professional appearance.
  • Ability to work independently without constant supervision.
  • Strong presentation skills.
  • Adept with a variety of multimedia training platforms and methods.
  • Ability to evaluate and research and implement training options and alternatives.
  • Proficient knowledge and use of Microsoft Office Suite and related program software, most importantly Microsoft PowerPoint.
  • Basic technology and computer skills.
  • Desire to serve other with compassion, excellence, safety, collaboration, and trust.
  • Able to pass background criminal and driving check, drug testing.
  • Must speak English.

Job Type: Part-time

Pay: $30.00 - $45.00 per hour
