Responsibilities:

• Provide visiting home health services to an assigned caseload including admissions, re-certifications and discharges
• Develop the Plan of Care in coordination with all healthcare professionals assigned to the case
• Provide documentation using the provided iPad within the required time frame
• Be knowledgeable in the use of the current OASIS assessment for all patient admissions, re-certification, transfers and discharges
• Provide skilled nursing and head-to-toe assessments to the adult and geriatric population. This includes the oversight of medication administration and wound care treatment as prescribed in the physician's plan of care
• Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan

Qualifications:

• Licensed Registered Nurse in the state of New York with at least 1 year of clinical experience; Home care preferred
• COVID Vaccinated
• Oasis experience is strongly desired
• Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care a plus
• Have reliable transportation and be willing to commute to provide the best in-home patient care.
• Willing to take advantage of our excellent training system comprised of in-house and field orientation and become an elite healthcare provider!
• Some rotating weekend days are required and one summer and one winter holiday.
• Willing to work one (1) winter holiday and one (1) summer holiday per year.

Pay: From $90,000.00 per year
