Job description
The nurse navigator in this program will work to improve the health status and collaboration between physical and behavioral health providers for seriously mentally ill clients in the adult mental health programs. This RN will collaborate with the treatment team to identify client’s high risk health practices and develop wellness plans to assist in changing behavior. The RN will provide individual and group education, wellness activities, health assessments, and screenings. The nurse will facilitate interaction with primary care and specialists as needed for individual clients by direct communication or serving as a resource for case management staff in their collaborative efforts. This position will also provide coverage for other nurse navigators in the out-patient department.Location: Crozer Health · 7845- Mental Health Case Management
