Position Description:
The Home Health LPN will provide intermittent care to Home Health and/or Hospice patients under the guidance of a Registered Nurse and according to an established physician Plan of Care, within the Licensed Practical Nurse scope of practice.
Education:
Minimum - Vocational / Technical - Graduation from an accredited school of practical nursing
Experience:
Minimum - 1 - 3 Years
Preferred - 3 - 5 Years
Skills:
Minimum - Analytical
Minimum - Organizational
Minimum - Verbal Communication
Preferred - Computer Systems (specify) - Cerner, other Home based software experience
Preferred - Ability to Multi Task and Prioritize
Licenses/Certifications:
Minimum - Active Licensed Practical Nurse (LPN) licensure in accordance with the Kansas Nurse Licensure Compact - Upon Hire
Minimum - Healthcare Provider CPR – (American Heart Association or American Red Cross ONLY) - within 7 days
