Job description

Transitions is currently expanding our clinical team, and seeking a Counselor for our intensive outpatient treatment program.

Responsibilities include conducting group counseling, client education, independent living skills and co-occurring disorders groups, preparing our clients for the maintenance phase of recovery. Counselor will perform daily charting in compliance with 245G requirements in Procentive. Hours are 8am-4:00pm Monday through Friday.

Transitions is set up like a campus, with housing for different levels of client care from board and lodging programs to sober housing, all within walking distance of Transitions Outpatient Program. We are located in Merriam Park, centrally located between Minneapolis and St. Paul. Desired skills:

• Demonstrated ability to provide culturally responsive and trauma responsive services to adults.

• Ability to utilize motivational interviewing concepts and interventions as well as de-escalation strategies.

• Experienced working with co-occurring disorders.
