Functions as a liaison between Montefiore Nyack Hospital inpatient/outpatient and Home Care in regard to receiving, facilitating and coordinating patient referrals to Home Care. Provides education and consultation to patients/families, hospital staff and physicians regarding Home Care program and services.
Qualifications
BSN Degree required.. Two years MED/SURG and two years Community Health Nursing experience. Licensure: Current license to practice as a registered professional nurse in New York State.
