We are looking to add Full Time, Part-Time and PRN RN Case Managers for Strongsville, Berea, North Royalton, Cleveland, Akron, Kent, and surrounding areas to our team. Area of coverage is determined by candidate's location.

$10,000 sign on bonus available for one year commitment (Full Time only), $20,000 sign on bonus available for two year commitment (Full Time only)!

RN Case Managers oversee the entire care of the patient beginning to end while under the care of Residence Home Care.

· Provide care wherever the patient calls home.

· Manage other disciplines such as LPNs, aides and therapy staff to give the best care needed for each patient.

· Ability to complete OASIS, recertifications, discharges, and admissions documentation within an EMR System. (Wellsky/Kinnser)

· Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.

Our RN Case Managers participate in a flexible on-call rotation with additional compensation.

· Attend meetings, CEUs in the North Olmsted, Ohio office on a regular basis

Requirements:

Working for a home health care agency requires the following:

· Current RN Ohio license in good standing.

· Reliable auto and current auto insurance

· Current CPR.

· Solid understanding of Medicare/Medicaid guidelines as it applies to Home Health Care.

· Home Health Care experience is preferred

Job Types: Full-time, Part-time, PRN, Per diem

Pay: $70,000.00 - $75,000.00 per year
