SPANISH SPEAKERS A BIG PLUS

We are seeking a compassionate and dedicated RN/LVN/CHHA to care for our patients in their homes. The ideal candidate will have excellent interpersonal skills to assist with interdisciplinary team member communications. This is a per visit job, NOT a per hour visit position.

Care Partners Hospice is currently seeking experienced and newly licensed Registered Nurses/Licensed Vocational Nurse and Certified Home Health Aide to join our team in the COUNTIES of Los Angeles/San Bernardino/San Diego. Specific areas may include: Lancaster/Palmdale, Victorville/Adelanto, Santa Ana/Orange, San Diego/Chula Vista/San Ysidro/National City.WE ARE WILLING TO TRAIN.

These skilled nurses/homehealth aides will be able to make visits within (10-30 miles distance) from their home to patient’s place of residence, in order to provide skilled care services as ordered by referring physician.

Must be able to cover most or all of the following areas:ADELANTO, VICTORVILLE, LANCASTER, SANTA CLARITA, PALMDALE, GREATER LOS ANGELES, SAN DIEGO, CHULA VISTA, SAN YSIDRO, SAN JUAN CAPISTRANO.

Per-Diem status (Skilled Nurses are able to choose areas of their choice).

Minimum Requirements include:

Graduate of anaccredited school of nursing
Must be willing to travel on weekends and nights, if needed
Must be open to work 4 days out of the week (Monday through Sunday)
Experience in software preferred
Provide skilled nursing services / interventions in accordance with physician orders
Assesses patient needs, plans for, evaluates and modifies care to meet goals of skilled nursing interventions.
Collaborates with all disciplines to plan and evaluate team goals for each patient.

Required Licenses and Certifications:

VALID Registered Nurse/Licensed Vocational Nurse/CHHA in CA
Current CPR certification
Valid Driver’s License and Auto Insurance
Physical/TB from the last 6 months or sooner
Pay: RN $80 to $250 per visit; LVN $45 to $60 per visit; CHHA $35 to $40 per visit. This is a PER VISIT JOB AND NOT PER HOUR.
