PROGRAM: Atlanta Healthy Start
The Center for Black Women’s Wellness (CBWW) is a community-based nonprofit organization that is committed to enhancing the health and well-being of Black women and their families. The organization provides several comprehensive programs that are designed to educate, empower and inspire participants to make positive changes in their lives and the lives of their families.
POSITION OVERVIEW
The Atlanta Healthy Start Initiative (AHSI) is a case management/home visitation program designed to improve maternal and child health outcomes in high-risk communities. Reporting to the Care Management Coordinator, the Family Support Specialist I is responsible for providing case management to the Atlanta Healthy Start program participants.
ESSENTIAL POSITION RESPONSIBILITIES
- Conducts risk assessments and screenings (ex. depression screening) with clients
- Works with clients on case plan development, reviewing and modifying periodically
- Utilizes home visitation curriculum and conducts structured, scheduled home visits with pregnant and postpartum clients
- Provides education and materials to clients on maternal and infant health-related topics
- Develops rapport with clients, while maintaining appropriate boundaries
- Maintains accurate and updated files for all assigned cases
- Inputs client data into database system in a timely manner and generates reports
- Refers clients to appropriate health and/or social service agencies (ex. WIC, family planning), and conducts follow up to track services received
- Attends monthly Community Action Network (CAN) meetings and reports on case management updates
- Identifies new referral sources and resources for clients
- Coordinates with Care Management Coordinator and Nurse Case Manager on health-related education
- Coordinates with Wellness Program Manager and Health Educator to identify and address health education needs of clients
- Assists in the coordination of client group meetings and activities
- Participates in ongoing training to remain current on maternal and infant health issues
- Performs other duties as assigned and necessary for successful program administration and delivery
EDUCATION
- High School Diploma or equivalent
QUALIFICATIONS
- One (1) year of case management work experience in a community, educational or medical environment
- Basic knowledge of Microsoft Office
- Knowledge of Metro Atlanta community resources
