Green Acres Healthcare Center is a Secured 84-Bed Skilled Nursing Facility that specializes in Behavioral Health. We are dedicated to meeting the physical, psychosocial and emotion needs of our residents. We provide Short-Term, Long-Term Care and Rehabilitation Services. Our commitment is to Facilitate the Healing and Recovery of our residents.

• Green Acres is unlike other Skilled Nursing Facilities, about 70% of our Residents are Ambulatory, Continent and can Feed themselves. Our residents require Phycological and Emotional Care not just Physical.

CURRENT SHIFTS AVAIALBE TO CHOOSE FROM:

Full Time: (PM) 2:00PM - 10:00PM

Full Time: (NOC) 10:00PM - 6:00AM

RESPONSIBILITIES: May include but are not limited to the following;

• Assist Residents with (ADL's) Activities of Daily Living
• Provide the best possible care for our Residents according to Care Plan
• Answer call lights to determine and meet Resident Needs
• Feed Residents as needed, Assist with Preparing Residents for Meals, Continual offering of Hydration and Nourishment. Report and Record
• Perform Daily Rounds and Check and Report any changes.
• Provide Companionship and Excellent care to patients to improve Quality of Life
• Maintain a Clean, Healthy and Safe Environment for our Residents and their Families
• Prioritizes patient/resident activities based on current care needs
• Administer Proper lifting and transitioning techniques for residents during positioning, wheelchair to bed, bed to chair, etc.
• Transporting residents to dining area for meals and activities
• Be punctual and professional and attend work when scheduled
• Work under the direct supervision of a LVN Charge Nurse or RN Supervisor

LICENSE: (REQUIREMENTS)

• CNA (required)
• CPR (required) (BLS)
• ***NON-Licensed Candidates may enroll in our Tuition Free CNA school, after a 5 week course you can begin employment at our Facility upon passing your State exams.***
