OBJECTIVE SUMMARY
: Responsible for providing drug therapy review/monitoring, prescribing, preparation, counseling, dispensing, and drug control in accordance with departmental and hospital policies and procedures as well as all applicable laws and regulations.


ESSENTIAL FUNCTIONS:

  • Responsible for providing quality pharmaceutical care to patients of Snoqualmie Valley Hospital. This includes all aspects of therapeutic selection, implementation (including prescriptive authority protocols), monitoring, modification and dispensing of drug therapy – all with appropriate documentation and communication.
  • Responsible for documenting drug information questions, drug utilization evaluation activities and adverse drug reactions.
  • Understands and reacts effectively to the unique needs of neonatal, pediatric, adolescent, adult, and/or geriatric patients as demonstrated by effective documentation in progress notes and pharmacy care plans.
  • Responsible for adherence by pharmacy interns, pharmacy residents and pharmacy technicians to departmental policies and procedures as well as State and Federal laws and regulations.
  • Promotes the adherence to the SVH Drug Formulary as approved by the Pharmacy and Therapeutics Committee of the Medical Staff.
  • Assures that proper charging documentation is completed correctly for services provided.
  • Promote and market Snoqualmie Valley Hospital and the pharmacy department to the community, patients, hospital employees, members of the medical staff and their office employees in an informal manner.
  • Maintain a positive, direct, open communication line with members of the SVH Medical Staff and their employees.
  • Maintain clinical competency through successful completion of the self-study modules and providing clinical services in practice.
  • Provide teaching, direction and supervision of pharmacy students, residents and medical assistants working under your direction.
  • Participates in and supports professional organizations.
  • Take an active, participating role in the development and implementation of new programs, while supporting and facilitating the programs in place.
  • Completes assigned competency assessments.
  • Completes additional responsibilities/projects when assigned by Director.

REQUIRED EDUCATION:

  • Pass oral and written examinations pertaining to the areas of current SVH clinical pharmacy service responsibilities.
  • Licensed as a Registered Pharmacist in the State of Washington.

PREFERRED EDUCATION: Completion of an advanced degree preferred, but not required. Completion of an ASHP Accredited Residency and/or Fellowship preferred, but not required

REQUIRED EXPERIENCE: Demonstrated experience in the following areas: Anticoagulation therapy, bridge therapy, TPN, Vancomycin dosing/monitoring, drug information, and demonstrated teaching ability.


PREFERRED EXPERIENCE:

  • Professionally ethical and experienced in directing and supervising pharmacy students, residents and medical assistants.
  • Demonstrated ability regarding bedside manner with patients and communication skills with patients, their families and health care professionals.
  • Current in prescribing trends, new drugs and drug treatment developments in the anticoagulation clinic setting.
  • Demonstrated skill in developing and maintaining positive interpersonal relationships with a diverse population.
  • Participates in and supports professional organizations.
  • Clinical pharmacy (2 -4 years) in a hospital setting
  • Ability to communicate and work effectively with the physical and emotional developments of all age groups.

SKILLS:

Eligibility Requirements - Employees regularly scheduled at least 20 hours per week.

Employee Dependents - Your legal spouse or domestic partner, Dependent children until age 26.
