The purpose of the Certified Occupational Therapy Aide is to coordinate an individualized program to maintain or improve the functional abilities of the resident’s activities of daily living (ADLs) utilizing the individuals' skills and team expertise.
- Qualified candidate must be fully vaccinated or have an exemption.
Essential Duties
- Apply therapy recommendation involving muscle strength, coordination, endurance, functional mobility, postural control, sensory awareness, functional balance, daily living skills and community integration per the treatment plan as defined by the Restorative Nursing Program.
Develop and plan program for restorative needs on admission, during and after skilled therapy, placing in active restorative or a functional maintenance program.
Writes measurable objectives/goals and interventions to be documented in the care plan and in the clinical record. Program must cover two areas of ADLs incorporating the resident's past and current status. Update goals and care plan as skills change.
Monitor record keeping and update programs. Goals limited to no more than 90 days without update.
Actively progress restorative program to a functional maintenance program, which becomes “the way we care for our residents”, enabling residents to achieve/maintain their highest level of functioning possible.
Plan individualized restorative exercise/ADLs and restorative groups of no more than four residents to one staff or supervising helper based on resident’s abilities and interests.
Provide constructive reinforcement and motivation to unit staff promoting consistency and perseverance to achieve the desired outcome for the restorative program.
Relate and communicate positively, effectively, and appropriately with the entire interdisciplinary team, residents and families, and members of the community.
Educate team on restorative techniques and approaches.
Attend therapy Medicare meetings, morning report, ICP quarterly and annual meetings to stay current on individualized programs for possible revisions.
Track and monitor daily minutes/feet, weekly Medicare progress and monthly achievement summaries. COTA completes restorative note under the direction and co-signature of the MDS nursing team.
Additional Tasks
- Train CNAs and other members of the resident's care team in the technique and tasks to promote resident involvement of the individualized program, "to be proactive-not reactive to their needs". Provide documentation of such training to Staff Development Coordinator upon request.
May provide wheelchair and/or walker on an initial admission to facility or if unit needs extra for transportation.
Provide or assess resident's need for different/additional assistive devices or equipment in keeping with the recommended therapy directed treatment plan, updating therapy should any new equipment be recommended.
Maintain and keep stock and inventory of wheelchairs and other assistive devices for the entire facility. Inform management of any needs in a timely manner.
Drive facility vehicle for home safety assessments with therapy department or restorative outing.
Report and document any incidents involving residents, coworkers, or visitors to appropriate personnel.
Follow infection control safety and standard practices upholding all policies and procedures related to infection control.
And other duties as assigned.
Minimum Training and Education
Required: Completion of an American Medical Association and American Occupational Therapy Association accredited training and internship recognized by the Wisconsin Medical Examining Board
American Occupational Therapy Board Certification
Obtain 12 Continuing Education Credits biennially
Preferred: Experience with the geriatric population
Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered.
