Remote Position
Summary
Conducts initial clinical assessment and financial verification of referrals from hospitals and other sources based on clinical and financial requirements. Determines optimal placement within portfolio based on facility clinical capabilities, location, bed capacity, and payor source.
Manages the transition of care from hospital to facility which includes reviewing and coordinating necessary resources for admissions, scheduling and transport.
This position may collaborate with insurance Case Managers and families. This position is supported by facility marketing, Admission and Case Management departments.
Qualifications:
- Associate’s or Bachelor’s Degree in Nursing
- LPN or RN preferred but not require if applicant has experience
- Minimum 2 years experience in health care setting or a related field
- Strong Organizational Skills, Good people skills, Ability to multi-task
- Customer services, sales and/or marketing experience preferred.
Knowledge & Skills:
- Ability to quickly learn new software applications and high level of comfort in leveraging technology for work
- Comfortable and accustomed to smartphone use, including but not limited to responding to and answering issues/questions from the field via text in a prompt manner
- Experience reviewing History and Physical information from Medical Charts, and fluent regarding medical terminology.
- Intermediate Level MS Office Skills
- Basic knowledge regarding Managed Care Levels of Care
- Ability to look-up and interpret insurance benefits coverage, and request authorization for SNF services.
- Strong organizational skills
- High level of customer service orientation including responsiveness and follow-through.
- Very strong oral communication skills. Ability to communicate effectively with external and internal customers. Ability to build trust and positive relationships.
- Exemplary ability to manage a heavy case load, with multiple simultaneous updates in real-time. Stays calm and focused on task while managing multiple tasks, projects, and/or assignments simultaneously.
- Ability to prioritize and think critically, and make sound judgment calls.
- Ability to evaluate all components of a SNF referral, consider financial and clinical risks, decide whether to accept or reject, and optimal placement within portfolio.
- Skilled in patient admissions assessment.
- Ability to work flexible hours.
- Ability to work independently with minimum daily direction, and to self-motivate.
- Ability to communicate effectively with patients and their families, physicians, hospital administrators, and at all levels of the organization.
- Knowledge of, or ability to learn, reimbursement programs.
- Ability to conduct oneself in a mature and professional manner.
Essential Functions:
- Promotes Client Facilities capabilities to referral sources and prospective residents/patients.
- Interacts with referral sources to generate a positive image and encourage word-of-mouth referral activities.
- Oversees communication with referral sources regarding referrals, and the acceptance or denial of referral. Manages the flow of information needed by the assigned facility, which includes providing facility constraints.
- Conducts initial assessment to verify and review coverage and benefits with carriers. Uses necessary tools to obtain information, which includes the electronic financial verification systems or discussion with various carriers.
- Coordinates the transition to assigned SNF including scheduling time of transition, initial medication orders for arrival, and equipment availability.
- Assists facility team with analysis that offers creative ways to optimize the facility’s ability to accept new patients. Notify Admissions Coordinator of any referral status changes and provide all information necessary to admit new patients.
