Oneida Health Hospital is looking for a full-time Case Manager/Medical Social Worker to work in our Care Transitions Department. This position is responsible for assisting the Care Transition team in providing education, assessments, and counseling to patients within our hospital clinical departments. The ideal candidate will have an empathetic approach to patients and work collaboratively with colleagues from other disciplines.

Your Essential Qualifications:

  Education:
  • Master’s in Social Work or Mental Health Counseling (Preferred)
• License:
  • New York LCSW (Required)
• Experience:
  • Clinical Mental Health: 2 years (Preferred)
  • Hospital experience a plus
