Job Description:

Principle Responsibilities

• Works collaboratively with patients to improve their health and well-being.

• Practices in a thoughtful manner including consideration for cultural diversity, individual autonomy and legal responsibilities.

• Actively participates in multidisciplinary team approach to case management.

• Utilizes quality improvement tools to develop processes that promote continuous improvement in delivery of patient care, patient outcomes, or workflows.

• May participate in peer review and quality assurance.

Job Requirements

• Master's degree Completion of accredited Nurse Practitioner program required

• 2+ years' experience post Nurse Practitioner training required

• Licensed Registered Nurse - MN Board of Nursing required upon hire and

• Certified Nurse Practitioner - ANCC - American Nurse Credentialing Center (ANCC) required Certification within specialty upon hire

• ACLS-BLS Tier 2 - Multisource required within 180 Days

• Drug Enforcement Administration upon hire required

Additional Job Description:

The Courage Kenny Advanced Primary Care Clinic Nurse Practitioner role is a position which will independently perform the role of Primary Care Provider for medically complex patients with stroke, spinal cord injury, brain injury, neurogenetic, neurodegenerative and congenital neurologic disorders, among other conditions.

• Manage comorbid conditions including diabetes and sequela, coronary artery disease and psychiatric illness, in addition to population specific complications, such as neurogenic bowel and bladder, spasticity (including knowledge of intrathecal pumps) and decubitus ulcers, all in the context of optimizing physical rehabilitation

• Extended visit times for complex patients allows for greater opportunity to get to know your patients

• Take call on a rotating basis, servicing both outpatient after-hours calls and those from transitional care unit

• Guide goals of care conversations for patients at end of life

• Collaborate closely with multidisciplinary team of nurses, social workers, and specialty physicians

• Follow patients’ hospitalizations and arrange close follow-up after discharge

• Treat patients both in-person and via telemedicine
