The Licensed Practical Nurse GS-7 serves as a member of the Restorative Nursing Team in the Geriatric and Extended Care setting within the Finger Lakes Health Care System. The GS-7 Lead LPN has additional patient care and administrative duties. They provide specialized care to Veterans with multiple disease processes on a regular and recurring basis.

Our company is seeking a highly skilled and experienced Licensed Practical Nurse for our long term care facility. The core responsibilities of this position include providing direct and indirect care to our geriatric patients, as well as assisting with the accurate recording of various medical information. The LPN works under the direction and supervision of the registered nurse and displays knowledge and ability in providing a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. The LPN serves as a responsible member of the nursing team and interacts with patients, family members, professional and supportive personnel who provide patient care, and with members of other services.

Our ideal candidate is compassionate, warm and reliable, and will have a background in geriatrics or medical-surgical nursing. A successful candidate will also have thorough knowledge of the federal regulations related to nursing.

The expectations of the LPN/ LVN include participation in data collected, planning, implementing and evaluating:
1. Maintaining positive patient/family relationships.
2. Assisting in the collection of objective and subjective data relative to more complex patient situations including:
a. Identifying and reporting subtle changes in patients' behavior or condition.
b. Identify and report variances related to neurological signs, l&O, IV flow rate, vital signs and other parameters.
3. Participating in patient centered conference/shift reports and contributing information for the development and modification of the patient care plan.
4. Anticipating a clean, safe and therapeutic environment and taking appropriate actions. Maintains & promotes safe waking practices in patient care.
5. Identifying the need for obtaining non-routine supplies and equipment.
6. Providing physical/psychosocial & spiritual support and instructions relative to diagnostic procedures.
7: Performing aspects of nursing care in a variety of situations to meet patients' needs for:a. personal hygieneb. physical comfortc. activity, exercise, restd. nutritione. eliminationf. prevention of skin breakdowng. rehabilitation aspects of careh. socialization
8. Anticipating emergent situations and initiating appropriate action.
9. Performing complex treatments relevant in a variety of situations.
10. Knowledge and skill sufficient to make judgments prior to preparing and administering medications and to recognize side effects and/or untoward reactions.
11. Assisting with monitoring prescribed therapies such oxygen and IV therapy.
12. Reporting patient status to a registered nurse and documenting information in the medical record including current status and/or subtle changes in patient's physical, psychosocial and spiritual status.

Job Type: Full-time

Pay: $56,516.00 - $73,469.00 per year
