Primary Location: Wayne & Oakland Counties, MI

Schedule: Full-Time & Part-Time available

Responsibilities and Duties

• Performs the initial assessment/evaluation and ongoing reassessments/re-evaluations to the client, determines the appropriate schedule of services for the client and communicates this frequency to the Clinical Supervisor.
• Communicates with the physician as needed to update client status, clarify and/or receive verbal orders.
• Develops and implements an individualized plan of care for each client, based on the assessment or evaluation, and in conjunction with the client, family, and physician.
• Delivers skilled care as outlined in the plan of care.
• Observes signs, symptoms and changes in the client’s physical or emotional condition and consults with the physician or authorized practitioner concerning alteration of the plan of treatment in consultation with the clinical supervisor, as necessary
• Teach, supervise and counsel the client and/or caregivers.
• Records client’s condition, interventions and evaluation in the electronic medical record as documentation of home visits, according to agency policy.
• Submits a record of visits via a patient signature log and clinician activity log weekly.
• Develops aide care plan and supervises HHA according to Agency policy, including instruction, demonstrations, and return demonstrations as necessary.
• Maintains client rights and confidentiality. Follows HIPPA compliance at all times.
• Occasional Weekend and Holiday coverage needed.

• Ability to effectively work with a diverse employee/client group.
• Ability to organize and manage time efficiently.
• Must be able to perform physical and social assessment of client and family.
• Excellent verbal and written communication skills.
• Demonstrate compassionate attitude and superior customer service skills.
• We are willing to train those with no Home Care experience.

Requirements

• Experience with providing patient care in their home environment.
• Valid Driver's License
• Proof of Automobile Insurance
• CPR/BLS Certification
• State of Michigan Clinical License: Registered Nurse
• Professional Degree from an Accredited University
• Current with TB Vaccination or Chest X-Ray
• Covid19 vaccination or valid exemption

Salary

• Negotiable, based upon experience.
• Signing Bonus available with 2 year contract commitment

Required Education:

• Registered Nurse (RN) Degree
• Associate's Degree (accepted) Bachelor's Degree (Preferred)

Experience

Nursing: 2 years (Accepted) Home Health Care experience (Preferred)

COVID-19 considerations: All staff are required to wear masks when seeing patients and are provided proper PPE equipment. Vaccination is not a requirement of staff.
