Salary Range: $20.00 min DOE

General Information:

This is a full-time position that reports directly to the Latah Recovery Center (LRC) Director of Recovery Services and works with officials at the Latah County Jail.

Viability of this position is dependent on the Case Managers ability to successfully implement the 2021 Idaho Department of Health and Welfare Re-Integration grant.

Summary:

The reentry case manager works in concert with recovery coaches to provide pre/post release support with comprehensive planning and connection to resources as outlined in contract to the Latah County jail.

The reentry case manager assists individuals with planning and resource identification and application prior to release. Resources available to participants at release include food, clothing, access to housing and medical care. Able to pass background check to work in jail. Must have excellent attention to detail, good organization skills and attention to detail.

Education:

B.A. in related field

Essential Duties and Responsibilities:

· Coordinate the development of project goals, timelines, evaluations, and routine reports with the Latah Recovery Center Services Director.

· Participating in any communications, site visits, and progress reports with IDHW.

· Case management as returning participants navigate treatment court phases

· Develop an individualized re-entry plan with participants.

· Connect with community resources, working with local agencies/peer support.

· Track participant progress using standard required formats.

· Promote quality improvement while integrating participants into center programs.

Recovery Center/Crisis Center Accountabilities:

· Attend all monthly peer planning meetings

· Work in conjunction with recovery coaches to help participants grow in recovery.

· Cultivate positive collaboration within the team, participants, jail and other major community partners in the re-entry program.

· Support growth and program development in all areas of the centers.

· Keep current documentation, reports and proposals.

· Other duties as assigned to meet contractual requirements.

Examples of Duties:

1. Direct client services

Intake, develop plans, crisis response

2. Case Management

Meet with clients, document status, assist clients accessing resources, referral to outside entities as appropriate

3. Program Development

Continual quality improvement, identify needs and develop new programs, work with the community to promote Recovery and Crisis Centers

Supervisory Responsibilities:

If appropriate the reentry case manager will supervise bachelor level non-licensed peer specialists and recovery coaches relative to their work in implementing the grant.

Job Relationships:

The reentry case manager will maintain regular published office hours, peer appointments, attend all regular staff meetings, appointed board committee meetings and board of director’s meetings. They will establish and maintain regular contact with the Executive Director, Rural Crisis Center Network Manager, board committee chairs, recovery and re-entry related community resources, and awareness of agency-wide issues. Knowledge of Electronic Health Record software such as WITS or ability and willingness to learn are required.

Ethical Considerations:

The Recovery Center believes in Harm Reduction approaches, and maintains a Safe Syringe Exchange, naloxone and condom supply.

· Confidentiality protocol applies to all interactions with participants and co-workers. Confidentiality involves maintaining secure consumer and internal agency information. HIPAA and other privacy regulations apply.

· The Case Manager supports both Centers policies, goals, and mission.

· Political and social awareness are necessary professional characteristics required by the unique nature of work in a recovery center atmosphere.

Exhibit a Good Knowledge of:

· Human services principles and practices.

· Rehabilitation and treatment programs, personality traits associated with the disenfranchised population, social and family systems, cognitive behavioral education, and case management methods.

· Computer proficiency for data entry, designing and tracking reports; MS Office programs and basic Excel; e-mail; and composing reports and letters.

· Willing to work non-traditional hours in a facility providing program facilitation and case management service. Hours may include evenings and/or weekends.
