Job description
The Placement Coordinator is responsible for processing child placement referrals and placement.
Locates placements for youth exiting from State Hospitals, acute care, PRTF & YRCII.
Accepts all referrals and thoroughly searches for placement options.
Research respite for children with higher needs for Foster Care placements
After locating the appropriate placement, notify the Reintegration team.
Assists data personnel with appropriate paperwork following placement.
Maintains contact with all child placement agencies and foster parents to develop and maintain positive rapport.
Ensures that active files are updated in a timely manner.
Willing to accept On-Call schedule and completes all paperwork in a timely manner.
Adapts to a flexible work schedule.
Completes all documentation necessary to ensure that appropriate placements are located for clients. This includes CMS, iConnect and on-call documentation.
Participates in all discharge meetings, when unavailable locates alternate person to participate.
Reports critical incidents to DFPS verbally within 12 hours, written within 24 hours.
Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP's), rules, personnel policies, and procedures; related accreditation and licensure standards; and federal, state, local rules, statutes, regulations, and all term of the contract between Saint Francis Ministries and DFPS.
Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
Ensures clients' rights are protected.
Is knowledgeable of and follows all safety procedures.
Other duties as assigned.
One Year child welfare direct service or related experience.
Bachelor's degree in Social Work or related degree from an accredited college or university.
Prefer Master's degree in Social Work or related degree from an accredited college or university.
Must be 21 years of age.
Must pass a drug screen, MVR, Criminal History and DFPS History Check, Centralized Background Check Unit Texas Health and Human Services Eligibility and any background checks deemed to be necessary.
FBI Fingerprint Check
Lifting requirements of 30-50 lbs.
Must have a valid driver's license, acceptable driving record and reliable transportation.
Mandated Reporter.
Demonstrate effective written and verbal communication skills.
Understanding of the state welfare system.
Requires knowledge of the mental and behavioral health care system and managed care.
Able to solve problems but also able and willing to seek assistance when necessary.
Basic understanding of adolescent development, treatment, and defense mechanisms.
Understands adolescent therapeutic techniques and how this impacts the client.
Independent discretion/decision making within the scope and responsibility of the position.
