onehome is looking for an Registered Physical Therapist (RPT) in the Corpus Christi, Texas area to work for our home health agency on a Full-Time basis. Physical Therapists evaluate, assess, and deliver services according to a written plan of treatment established by a physician. They are movement experts who improve quality of life through prescribed exercise, hands-on care, and patient education.

Responsibilities include but may not be limited to:

• Provide comprehensive assessment including data elements to evaluate patients level of physical functioning and need for physical therapy services.
• Establish a plan of care with physician input
• Establish/instruct home exercise program
• Observe and report relevant changes in the patient’s level of functioning
• Document all appropriate observations and treatments
• Complies with all policies and procedures including, but not limited to HIPPA Privacy rule
• Provides summary of skilled services and client outcomes to physician and company supervisor
• Listens and responds to any patient requests and concerns
• Accepts only those assignments for which they are qualified

Skills/Qualifications:

• Graduate degree or greater in physical therapy
• Licensed/registered in the state for which they are practicing in
