Job Summary:

To coordinate the patient centered medical home, collaborating with clinicians and multidisciplinary team members for Women’s Health and Obstetric patients. The Ob Navigator’s primary role will be as the point of contact for this unique patient group. They will provide communication, education and coordinate care with a heightened focus on exceptional care through the entire pregnancy episode. The OB Navigator will assure patient access and be a role model to ensure that quality care is provided.

Must foster a culture of patient and family centered care, service, quality and safety excellence, while improving patient experience, employee experience and community relations.

Required Experience & Skills:

  • Graduate from a Certified nursing program
  • Current and active Registered nursing license
  • At least one year of clinic experience
  • At least one year of experience with obstetric patient care.
  • Knowledge of equipment, and technology used in the nursing area.

Qualities and Characters:

  • Sensitivity to low income, vulnerable populations
  • Ability to work as a team member, maintains a professional relationship and positive attitude with other.
  • Ability to work with entities within the community
  • Displays enthusiasm toward the work and the mission of the organization

Certifications:
Colorado Register Nurse
BLS certification
Requirements and Conditions:
Drug screen is required
Background check is required
