The Nurse Practitioner [NP] is a registered nurse with advanced education and clinical competency that enables them to be primary care providers who, working collaboratively within a multidisciplinary health team, is responsible for providing direct comprehensive care to clients. The NP educates the client, family and caregiver by promoting wellness, preventing health care problems, maintaining current health status and intervening in acute or chronic illness. The Nurse Practitioner has the authority to practice through a collaborative practice agreement with a collaborating physician under the jurisdiction of the Department of Pediatrics and/or the Department of Family Practice and Community Medicine.
RESPONSIBILITIES:
1. Performs a comprehensive functional assessment of the client based on age and history. Included are assessing the physical, developmental and mental health status and psychosocial status of the client by means of interview, health history, physical examination and diagnostic tests.
2. Management of clients, which includes: performing comprehensive physical assessment, formulating diagnostic planning in accordance with medical protocols. Refer clients to clinics or subspecialties and community resources when appropriate. Act as a consultant with the physician.
3. Prepares, administers, implements and -evaluates the health care plan for those clients for which he/she is responsible. Evaluates the client's response to treatment.
4. Prescribes medication using the NP license number in accordance with the New York State Education Law relating to prescriptions and record keeping.
5. Participates in client education by providing opportunities for the individual and families related health issues.
6. Determines the effectiveness of the health and treatment plan of care through documentation of client care outcomes. Reassesses and modifies the plan as necessary to achieve medical and health goals.
7. Works with Nursing Supervisor to implement a system to monitor children with special medical needs.
8. Responsible for the operation of a smoothly functioning medical care delivery systems that stresses a deep concern for the clients' well being, clients' rights, education concerning health and disease prevention.
9. Consults with Medical Director, as established in the collaborative practice agreement. Meets with the Medical Director on a prearranged schedule to discuss problem cases and makes available new intake charts for review.
10. Acts as a liaison with the hospital divisions, clinics and affiliates.
11. Identifies and manages emergency or crisis situations.
12. Provides staff development and acts as resource person for ancillary staff.
13. Assists Medical Director in planning and development of seminars and other educational projects.
14. Seeks opportunities for professional growth through participation in continuing education and hospital committees.
15. Contributes to the education and implementation of agency medical departmental, State and City policies, procedures and standards.
16. Develop public relations materials to enhance community awareness of services.
17. Assist in the selecting of qualified candidates for the clinic.
18. Assist in the clinic scheduling for optimal utilization of staff to meet client care requirements.
QUALIFICATIONS:
Licensed, registered nurse in New York State
Certification as a Nurse Practitioner by the New York State Educational Department
