SUMMARY:
The Moms Do Care Program Manager will collaborate with the MDC Medical Director and multidisciplinary team to oversee the Moms Do Care program at LCHC. Moms Do Care is a statewide DPH administered program that focuses on caring for pregnant and parenting women with substance use disorder. At LCHC, we provide peer support services, prenatal care, addiction treatment, pediatric care, therapy and psychiatric treatment to pregnant and parenting women with substance use disorder. Our care is trauma informed, whole person, and comprehensive. Our goal is to nurture healthy families and enhance our participants’ autonomy.

At LCHC, the Moms Do Care program is run on Sunflower Team, a primary care team where we provide prenatal care, primary care, and pediatric care that is fully integrated with substance use and behavioral health treatment. We collaborate with care providers on the Obstetrics Team and throughout the health center as well as at Salem Hospital and in the wider North Shore community. Moms Do Care staff include people with lived experience who are in recovery, and people who have been specially trained to work with women who have substance use disorders.

The Program Manager will oversee the day-to-day functions of Moms Do Care including monitoring patient intakes and enrollments and ensuring appropriate data collection for funders, collaborating with Sunflower Team leaders to ensure appropriate access to care for MDC patients, providing supervision to the Peer Recovery Moms and CHWs assigned to Moms Do Care, collaborating with the team to identify and implement continuous quality improvement initiatives, and engaging with the wider community by running the monthly MDC-Lynn Recovery Council and ensuring effective communication with partner agencies.


Education/Experience:

  • Masters in Social Work, Public Health, or a related field required, LICSW preferred
  • Persons with lived experience of addiction are strongly encouraged to apply
  • Prior experience providing patient-facing care
  • 5+ years related experience in delivering community based services.
  • 5+ years of experience and /or training in substance abuse/behavioral health
  • Keep abreast of developments in the field and/or licensure through continuing education, participation in professional organizations or a combination of both. Knowledge skills and abilities:
  • Strong networks with community resources and the ability to create new relationships
  • Ability to work collaboratively within a multi-disciplinary team unit
  • Action oriented, able to identify and act on new opportunities
  • High energy and a sense of humor
  • Ability to overcome obstacles in a positive, productive manner; problem solving skills are critical
  • Experience with Trauma-Informed Care
  • Knowledge of HIPAA regulations and confidentiality requirements a must
  • Knowledge of healthy and appropriate boundaries when working with vulnerable and at risk populations
  • Familiarity with PCs and MS Office software applications
  • Ability to use EHR and data analytics tools
  • Strong communication skills, written and oral
