What we’re looking for in you

Required:

• Associate’s nursing degree from an accredited school
• Must have MN or WI RN license upon start date.
• Must provide proof of MN and WI RN license within 4 months of start, if both MN and WI RN licenses are not present on start date.
• Must have a dependable vehicle, valid driver’s license, auto insurance and meet all criteria required under Lakeview’s Automobile Liability Guidelines

Preferred:

• Proof of application for alternate state within 4 weeks of start, if both MN and WI RN licenses are not present on start date.

The skills/experience you bring as a Hospice RN Case Manager

Required:

• Minimum one year acute care setting experience or previous Long Term Care experience
• Knowledge of reimbursement procedures as well as knowledge of community resources
• Accept responsibility for maintaining current knowledge level of nursing trends in the Hospice field.
• Able to function independently with minimal supervision in home setting
• Strong assessment skills
• Ability to read, write and speak the English language
• Ability to carry out directions
• Computer experience
• Comfortable in end of life care
• Flexible with changing schedule
• Willing to participate in On-Call rotating schedule

Preferred:

• Homecare, Hospice or nursing home experience

The purpose of the Hospice RN Case Manager role

As an RN Case Manager you will develop, implement, and evaluate individualized patient care plans. They act as a, advocate for patient welfare, and serve as a liaison between patients, their families, and healthcare providers.

RN case managers work with new admissions, assessing their conditions and needs in order to develop personalized care plans. They regularly evaluate these plans of care and update them on an as-needed basis. They must educate patients and their families on how to follow their care plans.

RN case managers carry a case load as determined by Hospice Supervisor
