Homeless Assistance Program Coordinator/Case Manager
Job description
Youth & Family Services is in need of a Homeless Assistance Program Coordinator/Case Manager to coordinate and manage our Emergency Solutions Grant program, provide case management, and do other related support services for our Transitional Living Program and Street Outreach Youth Program.
Duties and Responsibilities are as follows:
Homeless Grant Funding:
- Screen potential clients for eligibility
- Provide appropriate referrals for callers as needed
- Complete and maintain files according to HUD and DOC guidelines.
- Prepare for and participate in annual DOC audits
- Conduct property inspections according to HUD and DOC guidelines
- Submit monthly/quarterly/annual reports, meeting grant reporting deadlines
- Prepare and submit material requests
- Develop and maintain a productive relationship with property owners who can assist in facilitating the program
- Adhere to strict client confidentiality policy
- Work with internal accountant to oversee program budget
- Enter data into HMIS/Service Point
- Participate in and provide support to Executive CoC meetings as needed
- Conduct and oversees PIT Counts for Canadian, Blaine, and Kingfisher counties
Case Management:
- Administrative duties to assist Program Coordinator to include data collections.
- Assist client with goals and implement service plan.
- Maintain accurate and confidential case files on each client; including progress notes, service plan and assessment forms.
- Provide youth advocacy, tutoring, case management and assist with educational and vocational counseling.
- Keep case files current and confidential.
- Coordinate with other referring agencies as to the client’s needs for other services.
- If sessions are conducted on school property, do so under the direction and cooperation of the school staff in a manner that will not interfere with other major scholastic activities and classes.
- Attend staff meetings of agency as directed.
- Facilitate/Assist with education groups as requested.
- Follow Policy & Procedures as set forth by YFS Board of Directors and memos from the Executive Director.
- Make available, in a timely manner, to data entry personnel, appropriate forms and records needed to compile agency reports and statistics.
- Be civic minded in your community catchment area. Be willing to speak to groups of community members in reference to specific areas of concern for youth and families.
- Attend seminars and workshops as required or as available. Stay informed about research and related service approaches.
- Employees may not transport clients or conduct agency business in a vehicle that is not covered by liability insurance. Employees shall also have current driver’s license issued by State Department of Public Safety. A current copy of automobile insurance must be kept on file at the administrative office in each employee’s record file.
- Perform other duties as directed by supervisor and/or Executive Director.
EDUCATION:
Graduation from an accredited high school or a GED equivalency. Associate of Arts or BA/BS is preferred.
EXPERIENCE:
Experience in social work, youth oriented programs, or child/youth care worker is preferred. Minimum age of 21 is required.

Pay: From $32,000.00 per year
