About Our Foster Care Program:
The Family Foster Care Program is a foster boarding-home program whose staff and foster parents are highly skilled and specialized in providing therapeutic intervention, case management and care coordination, skill building, and independent living for children and youth who have experienced trauma as it relates to being removed from the home due to abuse and/or neglect. This program is designed to provide clinical and case management support to children, their biological families, and the foster families who participate in the program. The program goal is to provide the level of care and support that would enable children to be successful in a community-based setting. The primary purpose of the program is the prevention of institutionalization for these children.

Family Foster Care Therapist Position Summary:
The Family Foster Care Therapist is primarily responsible for providing direct therapeutic and supportive services to children and their birth families. The Family Foster Care Therapist provides support and guidance to the foster parents and case management services to enhance the success of the placement. The Family Foster Care Therapist will have responsibilities in the Syracuse and Fulton areas.

This is a community-based position which will allow for more flexibility throughout the workday!

Job Functions:

• Leads and facilitates the development of the optimal functioning of the treatment team
• Develops and implements treatment plans for children and families
• Provides long-term therapeutic counseling to children, adolescents and their birth families, as appropriate
• Supports the foster families by providing guidance, information, and other services, as needed
• Maintains and updates individual client case records and foster family records and accurately documents services provided, utilizing the CONNECTIONS computer system
• Responds to and effectively handles emergency situations that may require flexibility of both time and areas of assignment
• Utilizes clinical interventions to support and assist program participants in meeting the goals of their service plan and achieving the maximum level of permanency
• Advocates on behalf of children and their families to ensure adequacy of services (i.e. educational, legal and community) by providing linkages and coordination with the county providers and agencies

Family Foster Care Therapist Qualifications:

• Masters in Social Work or related clinical degree and one (1) year post-graduate experience in social work practice with children and adolescents
• Prior related experience may be substituted for one (1) year post-graduate requirement
• Must have a valid New York State driver's license and access to reliable transportation and be willing to transport children when needed.
