Position Purpose:

The Case Manager supports young adults and their children as they transition from homelessness to permanent housing through the Rapid Re-housing Young Adult Program (RRH-YA). Duties includeorienting all eligible participants to the program and providing supportive services to promote participant's self sufficiency, integration into the community, and permanency in housing. The principal duties are performed both in the office and in the field and community where program participants reside. The Case Manager conducts assessments, determines subsidy eligibility, develops individualized case plans, links families with community-based housing and service resources, helps families manage their budget (security deposits, rent, etc.), and provides light-touch case management services in conjunction with community partners. The Senior Case Manager supervises the Housing/Employment Specialist (HES) and collaborates with the HES to maximize participant's ability to increase income and maintain permanent housing. Further the Senior Case Manager develops plans and ideas to further enhance the program as it grows.

Essential Duties and Responsibilities:

  • Conduct comprehensive assessments, the HAT (Homelessness Assessment Tool) of each head of household participant and their family within seven (7) business days of admission to the program, which must be uploaded in HMIS. Complete an updated HAT in HMIS annually from the date of the head of household's {HOH) entry and exit from the housing program.
  • Develop a written individual service plan (ISP) within 14 business days of the families' entry to the program and update the plan for each participant and their family using the comprehensive assessment assigned by the County, currently the HAT (Homelessness Assessment Tool) within 14 business days of the household's entry into the program, and update the plan every 90 days, which specifies the goals and interventions required by the Head of Household or for the case manager within specific periods, to address any barriers to maintaining permanent housing.
  • Maintains direct contact with each family, at least monthly.
  • Develop and maintain a model that is essential to assisting young families in becoming financially independent.
  • Assist household members to maintain stable housing, through ensuring that they maintain:

1) Consistent rental and utility payments, 2) clean and sanitary unit conditions and 3) positive relationships with neighbors and property staff, etc.

  • Provide household members with information and referrals to programs for emergency needs and other identified needs to include necessary medical care, behavioral health programs, and other services.
  • Facilitate regular groups that will assist young families in gaining life skills, developing budgets and becoming financially independent.
  • Provides supervision and management of the Housing / Employment Specialist (HES), consisting of direct oversight which ensures that the HES, is effectively assisting families with receiving support that will assist them in meeting housing and career goals necessary to gain financial independence and self-sufficiency, prior to exiting the RRH-YA program. Further ensures that the HES is meeting the tasks outlined in his/ her job description.
  • Provides supervision and oversight of the management and tracking of rental subsidies and ensure that participants have paid their portion of their rent and all utilities each month, which is managed by the HES.
  • Ensures that the HES collects all necessary documentation needed to support program eligibility and rental calculation.
  • Supervises and collaboratively works with the HES to find permanent housing and employment options for participants.
  • Provides management of the HES, who interfaces with landlords whenever necessary to mitigate issues or advocate for participants.
  • Provide case management services including, but not limited to: providing appropriate resources, tools, and counseling to assist participants in achieving their case plan goals; making referrals for services; following up to ensure that the referrals were completed and tracking and documenting participant progress.
  • Facilitate service planning meetings (every 90 days) to assist family with developing goals, review progress and develop plans to assist family with achieving goals.
  • Collaborates with HES to verify continued program eligibility, ensures re-assessment of participant's income and re-calculation of rent every 90 days.
  • Collaborate and communicate closely with formal partner agencies (i.e. Arbor and Montgomery Works etc.) to maximize participant outcomes, program goals, and agency mission.
  • Document and maintain up-to-date information on services provided to participants in the Homeless Management Information System (HMIS) and hard copy case records within established program timeframes.
  • Ensures that client files are maintained according to Agency, County, State, Federal and COA regulations.
  • Constructively utilize formal and peer supervision to evaluate and improve effectiveness in providing services to participants. This includes active participation in supervision, team meetings and agency meetings.
  • Provides weekly supervision to the HES, to ensure implementation of services and progression of case goals.
  • Attends weekly supervision with the program Director to discuss program plans, ideas, progress and challenges.
  • Provides a monthly written report to the Director of the programs progress, challenges and plans.
  • Participates in Leadership Meetings.
  • Maintain state licensure at the highest possible level given degree and experience.Non-Essential Duties and Responsibilities:

Qualifications:

  • Minimum licensed graduate Social Worker (Master's Degree and licensure at the graduate level)
  • Minimum of one year of professional experience in the social services field,
  • Computer proficient
  • Must have a good working knowledge regarding families
  • Must have a valid driver's license, a car to be used for work related tasks, and auto liability insurance.
  • Knowledge regarding family dynamics.
  • Demonstrates cross-cultural competencies.

Physical Demands/ Work Environment:

While performing the responsibilities of the job, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers to handle or feel. The employee is occasionally required to stand, walk, reach with arms and hands, climb or balance, and stoop, kneel, crouch or crawl. Vision abilities required by this job require close vision while working with a computer monitor. While performing the duties of this job, employee is occasionally exposed to moving mechanical parts and vehicles. The noise level in the work environment is usually quiet to moderate.
