HonorHealth is a non-profit, local community healthcare system serving an area of 1.6 million people in the greater Phoenix area. Responsibilities
Job Summary
The Palliative Care chaplain provides specialist level expertise related to the spiritual and existential needs of patients, families and staff. The chaplain provides comprehensive screening, assessment and interventions, including counseling, crisis intervention, education, and staff debriefings. The chaplain is a fully integrated member of the Palliative Care team and contributes to team goal setting, program development and quality improvement initiatives. The chaplain collaborates with the treating team in providing culturally appropriate patient and family-centered care.
Completes a spiritual assessment to identify sources of strength, meaning and support, existential concerns, cultural norms and preferences, concerns related to end-of-life.
Contributes to the spiritual aspect of the care plan based on the needs, goals, and concerns identified by the patient and family, recognizing and maximizing patient and family spiritual strengths.
Chooses and utilizes appropriate interventions and counseling techniques targeted toward the intended outcomes.
Provides grief counseling and bereavement support.
Helps to identify and interpret cultures and faith traditions that impact healthcare decisions.
Provides resources including prayer, sacramental ministry, rituals, life review, and connections to community clergy when necessary.
Assists in complex goals of care discussions, family meetings and other conversations regarding goals, values and emotional/spiritual/cultural needs.
Provides education on spiritual needs of patient and family to other healthcare team members.
Complies with standards for timely submission, accuracy and completeness of documentation in the medical record and collects all required statistical data.
Participates in quality improvement, team goal setting and program development initiatives.
Performs other duties as assigned.
Qualifications
Education
Master's Degree in Divinity or its theological academic equivalent Required
Experience
1 year experience providing spiritual care in an acute care setting and experience in palliative care or hospice setting. Required
