ProMedica Hospice, formerly Heartland Hospice is searching for a part-time Social Worker to support their patients and families in the Madison and surrounding areas. This is a weekend position working 8am-8pm Saturday and Sunday.
The Social Worker is responsible to provide clients and families psychosocial care regarding economic, social and emotional needs. This position requires WI CSW.

Educational Requirements
Bachelor's Degree in Social Work. May require licensure or certificate as required by the state. Must be in good standing in the state of practice.
Position Requirements
100% travel necessary on a daily basis. Experience in varied multidisciplinary settings in the health care field. Experience working with patients and families coping with terminal illness and death. Excellent communication and interpersonal skills.
