Responsibilities:

• Develop a rapport with clients.
• Help clients and their families understand the illness and the treatment plan.
• Evaluate the client's progress and adjust treatment plan, if necessary.
• Provide patients with counseling, education, support, or referrals as needed.
• Report clinical observations and client progress to the doctor at regular intervals.
• Plan activities to enhance the well being of patients.
• Deal with insurance companies and other medical agencies to help patients obtain medical care and financial assistance.
• Document all data, notes, and reports.
