Overview

As our Social Worker at CHI Health Immanuel Child/Adolescent Inpatient Psychiatric Unit, you’ll perform social work assessments and interventions for patients. The average length of patient stay is 2 days. Working collaboratively with a nurse case manager, you will develop and implement treatment plans that support patient-centered care for individual patients and the medical community. Being dedicated to patients and family-centered care, you value the personal self-determination of your patients. To succeed in this role, you must manage multiple priorities simultaneously and engage in creative, compassionate, and ethical problem-solving.

Responsibilities
- Perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations
- Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families.
- Serve on hospital committees and develop and present in-services to Hospital staff.
