Social Services - LCSW (Licensed Clinical Social Worker)

13 - Week Contract Position

4/24/2023 - Anticipated Start Date

Mid 3x7.5-Hour (14:00 - 20:30)

• Social Worker in Care Management - Part Time Swing Shift 1400-2030 + additional shift with varied times 0830-1700 or 1700-2300 - Weekend Flexibility Required

***Must have acute hospital, ER, and psychiatric experience. Social work and discharge planning.***

1 year minimum experience required!

Department Details:

• SWCM work as part of multidisciplinary team with a focus on psychosocial issues, end of life issues, homelessness, substance dependence and discharge planning
• Daily Patient Load: 22-26
• Patient Age Group: Adults, Geriatrics

• Emergency Detention
• Adoption/Foster Care
• Suicide/Homicide/Self-Harm
• Coordinate Services - Home Care
• Documents Information to Facilitate Care Coordination
• Participates in Care Conference Activities
• Refers to Rehab and Long Term Care Facilities
• Refers to Other Disciplines/ Resources
• Supervises Staff Effectively and Efficient
• Documentation According to Plan of Care
• Documentation of Care Delivery, Plan, Goals, etc.
• Documentation of Patient/ Caregiver Teaching/Learning
• Documentation Reflects Understanding of Coverage Criteria
• COVID-19 Vaccine (Facility Guideline): Required + Booster - No Exemptions, Current Booster
