This position may be eligible for the Education Debt Reduction Program (EDRP), a student load payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
The Erie VA Medical Center Community Living Center (CLC) provides 24/7 compassionate, Veteran-centered care in safe and home-like environment to eligible Veterans who require nursing home level of care. This is a 22-bed unit that provide in-center care for the Veteran residents housed in the stand-alone building on the Erie campus. This unit serve the needs of Long-Term Care and Hospice residents.
The goal of care is to restore function, prevent decline, maximize independence, and provide comfort during end-of-life care. The goals of care are achieved in an environment where the Veteran is respected, treated with dignity, and invited to be an active participant in his/her care. The RN will be responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs. The role requires support and active participation in initiatives such as High Reliability Organization (HRO), Patient Centered Care, Just Culture and I CARE.
Duties
Duties include, but are not limited to:
· Provides nursing care and continuous assessment based on written nursing standards of care, policies, and orders by the provider.
· Responsible for assessment, documentation, intravenous access, obtaining EKG, bloodwork, baseline vital signs and carrying out all orders.
· Responsible for assessment, pain management, obtaining vital signs as ordered, discharge instructions, and resident education.
· Administers oral and intravenous medications as ordered by provider.
· Promotes and maintains effective pain relief, comfort management, and emotional comfort.
· Conducts initial and ongoing assessment and documents per the CLC documentation guidelines.
· Utilizes Quality Improvement driven processes to ensure quality outcomes for the following: wounds, pain, restraints, falls, psychoactive medications and others as indicated.
· Other duties include, but are not limited to answering the telephone, obtaining specimens, paging physicians, paging ancillary departments for services, reviewing physician orders, facilitating caring orders out, printing lab labels and patient wristbands taking verbal orders in an emergency, assisting with transportation, coordinating patient bed flow, checking for expired supplies, and cleaning non-critical reusable medical equipment.

Qualifications
To qualify for this position, applicants must meet all requirements.
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Written and Spoken
Physical Standards: All applicants must meet the physical standards for the position. A physical examination prior to employment may be required.
Licensure: Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC)] at the time the program was completed by the applicant. Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
Preferred Experience:
· Geriatric extended care experience
· 1-3 years of RN experience
· Strong organization and independent judgement skills
· Ability to communicate and work effectively with staff, patients, families, and interdisciplinary groups
