Job description
All Metro Healthcare, a Modivcare Personal Care Service, is looking for (2) RN Clinical Supervisor to assist in assessing, planning, implementing, and evaluating the nursing process through All Metro Health Care policies, procedures, and standards. You will report to the Director of Clinical Services.

You will...

  • Performs on-site supervisory and teaching visits to evaluate professional and paraprofessional staff on assigned cases and provides follow-up information to the Director of Clinical Services and physicians.
  • Provides clinical orientation to employees on assigned home care cases.
  • Counsels patient/caregiver in meeting nursing and related needs.
  • Performs initial nursing assessments as designated by the Director of Clinical Services and assists in assigning qualified personnel.
  • Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs.
  • Develops a multidisciplinary care plan for implementation by utilizing and modifying the nursing process as necessary.
  • Assigns and coordinates services provided.
  • Prepares clinical progress notes and reviews documentation, ensuring compliance with All Metro Health Care documentation standards.
  • Reports any changes in the patient's condition or needs to the Director of Clinical Services and physician.
  • Attends case conferences, staff meetings, and in-service programs.
  • Ensures adequate home care medical supplies and equipment are available on assigned cases.
  • Adheres to agency and patient care policies, including the implementation of patient rights and responsibilities.
  • Initiates appropriate preventative and rehabilitative nursing procedures.
  • Administers medication and treatments as prescribed by the patient's physician and the state's scope of professional nursing practice.
  • Monitors clinical records for complete and accurate documentation and adherence to policy.

  • NYS Licensed and currently registered as a Registered Professional Nurse.
  • Graduate of an approved school of nursing and holds a Diploma, Associate, or Baccalaureate Degree
  • One-year experience of Nursing experience in Home Care and one year of supervisory/management experience

Pay: The salary range for this full-time and exempt position is $74,880 to $91,520, not including variable compensation. An employee's exact starting salary will be based on various factors including but not limited to experience, education, training, merit, location, and the ability to exemplify ModivCare's core values.”
