Optimal Care, Inc. is a 4 Star Rated Medicare Certified/CHAP Accredited home health care agency seeking contingent and full time LPNs, Registered Nurses and Nurse Practitioners to provide home health services to patients in our service area. Home care experience preferred by not required. We are looking for highly professional, self motivated RNs and NPs who love working with the adult and geriatric homebound population. High Compensation in return for Clinical Excellence!

Territory: Oakland County, MI

Qualifications

• Successful completion of nursing program accredited by National League of Nursing preferred.
• HCHB experience preferred
• Licensed to Practice professional nursing in the state
• Minimum of two years of nursing experience preferred.
• Previous Home Health experience preferred
• Ability to travel during business hours

Responsibilities

• Provides initial and ongoing comprehensive assessments of client's needs and other services requiring substantial and specialized nursing skill.
• Coordinates the total Plan of Care and maintains continuity of client care by liaising with all medical professionals involved in client's care.
• Timely responses to inquiries and concerns from patients and their caregivers/guardians.
• Develops, prepares, and maintains accurate and timely individualized client care progress records.
