Overview:
The George J Otlowski, Sr. Center for Mental Health Care is recruiting a full-time Partial Care Clinician. Located in Perth Amboy, NJ, the center provides behavioral health services to the Middlesex County community and is one of the largest outpatient behavioral healthcare centers in the area. Our behavioral healthcare services are designed to help with a variety of psychiatric conditions and co-occurring disorders. Our multi-modality approach consists of individualized treatment planning that addresses patient’s needs and sets realistic therapeutic goals so that each individual who receives care achieves recovery and rehabilitation. Our team includes a medical director, psychiatrists, psychiatric nurse practitioners, registered nurses, psychologists, social workers, social work interns, interpreters and support staff.

Partial Care Clinician

Reports to: Partial Care Program Director and Assistant Director

Responsibilities:
  • Provides direct psychotherapy and assessment services to clients primarily in the group modality but including individual and family therapy as need.
  • Develops individual recovery plans and conducts reviews throughout the duration of treatment, with the goal of achieving clients' chosen valued roles and meeting discharge criteria.
  • In collaboration with their supervisor, abides by the standards set forth by the Division of Mental Health and Addiction Services and Medicaid.
  • When appropriate, clinically supervises staff assigned to them by the program Director and reports administrative concerns regarding their supervisees to same.
  • Follows the philosophy of illness management and recovery when creating plans for and working with clients.
  • Adheres to the program's mission and philosophy and the Center's policies and procedures. Observes privacy and confidentiality rules and respects the rights of clients.
  • Ensures Medical Unit secretaries schedule and reschedule clients' appointments with prescribers to ensure clients are seen as often as needed.
  • Arrange for transportation as necessary, linkage with outside resources, referrals to appropriate services as is needed by clients.
  • Completes any other tasks relevant to their program or role as determined by their direct supervisor.

SUMMARY OF DUTIES:
Assessment: (conducted by Master’s level clinicians)
  • Conduct intake evaluations that provide an overall assessment of clients that include all required areas of current and past functioning and psycho-social stressors, chosen valued role and discharge criteria and that information from the intake assessment is synthesized into an integrated summary that provides a total picture of the client. Formulates clinically appropriate diagnoses and develops recommendations for treatment.

Individual Recovery Planning:
  • Complete initial and updated individual recovery plans, reviews, discharge planning and case closings in a timely and clinically appropriate fashion.
  • Maintains accurate, high-quality client records, including but not limited to, Intake Assessments, Progress Notes, Individual Recovery Plans/ Reviews, Transfer Summaries and Closing Statements ensuring they are concise and well-written so that they contain good grammar and are grammatically correct.

Psychotherapy / Counseling:
  • Provide clients relevant and clinically competent psychotherapy/ counseling services that reflect appropriate planning and evaluation of care.
  • Conduct treatment in a variety of modalities including individual, group and family as indicated.
  • Assumes primary responsibility for case management of each assigned case; assists client in obtaining such other services as required and makes appropriate referrals within the Center and to providers outside the Center.
  • Demonstrates ability to use procedures for assessing and managing suicide risk.

Documentation:
  • Ensures that the clinical record is complete with all contacts with clients, or others on behalf of the client, as well as all client activities, services, status updates, and referrals provided in accordance with program policies and procedures, along with client's response and ultimate outcome.
  • In collaboration with clerical staff, all documentation in clients' electronic health record required by third party payors is maintained in the clinical record and that communication with such parties is timely, ongoing, and cooperative.

Performance Improvement/Productivity:
  • Works to ensure program's average daily attendance is at an optimal productivity level by consistently engaging clients to ensure their regular attendance in program.
  • Completes and/ or participates in all assigned performance improvement activities and attends performance improvement meetings and training sessions as appropriate.

Adherence to Rules and Regulations:
  • Adheres to the Center's mission, vision, and treatment philosophy in all endeavors and interactions. Follows the Center's policies and procedures, and all appropriate professional and regulatory guidelines.
  • Maintains all required clinical competencies.
  • Complies fully with infection control, emergency preparedness plans, fire/ disaster plans, hazardous materials plans/ MSDS information sheets, and environmental care plans.

  • Can effectively collaborate and communicate upward, downward and laterally in order to provide a seamless continuum of care and promote a harmonious work environment conducive to quality care.
  • Is a team player with a cooperative and harmonious disposition, with a willingness to share ideas, best practice techniques, and new ways of doing things.
  • Engages in supervision in a productive and appropriate manner; exhibiting a flexible therapeutic style and the ability to relate to various clinical perspectives.
  • Demonstrates flexibility and takes initiative with regard to meeting staffing needs of the program/ Center (e.g.: provides coverage for others).
  • Responds to clients' needs and concerns promptly and with compassion and understanding.
Qualifications:
Education: Completion of a Master’s degree from an accredited college or university in psychology, counseling, social work, or a related discipline.
Experience: Prior related experience in a clinical setting desired.
Language: Bilingual Spanish desired.
Knowledge, Skills, and Abilities: Demonstrated ability to work respectfully and productively with staff and clients. Possesses personal and professional commitment to, mental health practice principles and ethics and the Center's mission, and vision policies. Ability to adhere to set deadlines for documentation and maintain patient confidentiality. Demonstrates self-awareness, sensitivity to others, and the interpersonal skills needed to relate to a wide range of individuals, groups, and families.
