Clinician Patient Care Coordinator

Welcome! Capitol Hill Consortium for Counseling & Consultation LLC is a well-established and continually growing multicultural and multi-specialty mental healthcare practice, serving the Greater Washington DC region with affordable and insurance-based behavioral healthcare.

We are seeking a skilled, hard-working, goal-oriented individual to join our Administrative Team for an on-site position at our Washington, DC office.

Our Mission: We work together as a team to provide high quality, accessible, culturally-sensitive mental health service, training & consultation.

Responsibilities and Duties

  • Maintain clinician caseload
  • Provide contact information/maintain contact with clients of assigned clinicians
  • Return phone calls, emails and texts from new and existing clients within 24 business hours
  • Maintain clear partnership with clinicians to navigate requests
  • Maintain knowledge of external resources and referral sources to provide clinicians
  • Maintain updated training on InSync to remain knowledgeable and able to answer questions about Insync
  • Assist assigned clinicians with case management care
  • Oversee assigned clinicians’ schedule to ensure continuance of recurring appointments
  • Manage any permanent changes to clinicians’ schedules
  • Adds blocks in clinicians’ calendars for scheduled time off/vacations/etc.
  • Manage client discharge processes for all clients discharging from care with assigned clinicians
  • Schedule recurring meetings with assigned clinicians in order to identify intake availability, troubleshoot client issues, address clinician concerns, and maintain fluid communication
  • Notify clinicians before putting new clients on their schedule
  • Fill holes in clinicians’ schedules 2 days in advance
  • Coordinate with fellow care coordinators on internal transfers between assigned clinician(s) and next clinician
  • Inform managers when problems arise that need immediate direction
  • Keep the Intake Coordinator updated with clinician availability
  • Maintain professional and pleasant demeanor in all communications/interactions with clients and fellow employees
  • Be responsive and helpful to all clients
  • Assist with other admin duties as needed

Candidate Qualifications and Skill Requirements

  • Bachelors Degree from an accredited college or university (Preferred)
  • Experience working in a medical/mental health environment
  • Take initiative
  • Be detail oriented
  • Be able to manage multiple projects and a multi-phone line system
  • Have warm, engaging & attentive customer service skills
  • Have a positive working attitude with a solution-focused approach to conflict
